Job summary

Hillcrest Surgery is looking to recruit a Practice Nurse (or Nurses) to cover 4 days.

We are open to 1 Practice Nurse working 4 days but also welcome applicants looking to work 2 days (and we would recruit 2 new Nurses). We are open to all levels of experience and can offer training and development.

The Nursing/HCA team is currently made up of 3 part-time Practice Nurses and 2 HCAs.

You would be working as part of a supportive multidisciplinary team.

Main duties of the job

The main duty is to work as part of a team to provide primary care nursing services to our 8500 patients.

As well as cytology, wound management, and childhood and travel vaccinations there is the opportunity to use chronic disease management skills (or develop them) for a range of disease areas.

Nursing staff at Hillcrest Surgery support the Healthcare Assistants and work alongside Clinical Pharmacists, GPs and GP Trainees.

About us

We are a busy training and teaching practice with a clinical team comprising of 8 GPs, 3 Practice Nurses, 2 Clinical Pharmacists and 2 HCAs offering GMS and additional services to 8,500 patients.We normally have 2-3 GP Trainees.

The Practice will be moving to a new purpose fitting out premises in late 2024. The new premises is 500metres from the current site (King Street, Acton).

Job responsibilities

Practice Nurse

Role Summary:

  • To be responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population.
  • The practice nurse will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, infection prevention and control, as well as supporting the management team in the reviewing of clinical policy and procedure.

Primary Responsibilities:

  • Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on the patients health
  • Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
  • Provide vaccination and travel medicine services
  • Provide wound care (ulcer /Doppler etc.) to patients
  • Request pathology services as necessary
  • Process pathology results as required
  • Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required
  • Maintain accurate clinical records in conjunction with extant legislation
  • Ensure read codes are used effectively
  • Maintain chronic disease registers
  • Implement vaccination programmes for adults and children
  • Prioritise health issues and intervene appropriately
  • Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice)
  • Support the clinical team with all safeguarding matters, in accordance with local and national policies
  • Deliver opportunistic health promotion where appropriate
  • Manage emergency drug kit
  • Along with Nursing colleagues ensure adequate stock of vaccines and clinical consumables

Secondary Responsibilities:

  • Implement and evaluate individual treatment plans for chronic disease / frail patients
  • Chaperone patients where necessary
  • Support the team in dealing with clinical emergencies
  • Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
  • Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
  • Monitor and ensure the safe storage, rotation, and disposal of medicines
  • Support junior members of the nursing team, providing guidance when necessary
  • Participate in local initiatives to enhance service delivery and patient care
  • Support and participate in shared learning within the practice
  • Continually review clinical practices, responding to national policies and initiatives where appropriate
  • Participate in the review of significant and near-miss events applying a structured approach i.e. root cause analysis (RCA)
