Job summary

We are looking for2 Care Navigatorsto join our dedicated reception team on a permanent basis. The ideal candidate will be enthusiastic individual who enjoys working with general public in a fast paced environment. You need to be able to work as part of a well establish and close team but use your own initiative where required, within the remit of your role. Exceptional communications skills, both face to face and over the phone are a must as is ability to remain calm under pressure.

Main duties of the job

The specific requirements for these roles are as follow:

Post 1. Fixed Term contract for 12 motnhs (to cover maternity leave). There may be scope for role to become permanent.

Hours of work

Total: 31.5 hours a week to be worked as follow:

Monday 8.00-18.30 (with 30 minutes unpaid lunch break) main site

Tuesday 8.00-13.30 (Cotmanhay branch)

Wednesday 8.00-18.30 (with 30 minutes unpaid lunch break) main site

Friday 8.0-14.00 (main site)

Post 2 (Permanent)

Hours of work

Total : 27.5 hours a week to be worked on:

Monday 8.00-14.00- Old Station; main site

Tuesday 13.00-18.00 (Cotmanhay branch)

Wednesday 8.00-13.00 (Krk Hallam branch)

Thursday 13.00-18.00 (Cotmanhay branch)

Friday 8.00-16.00 (with 30 minutes unpaid lunch break) main site

The successful applicant will be part of a rota to cover late finish during the week, therefore flexibility is a must.

Ability to cover annual leave and a sick leave (at short notice at times)

Ability to cope with stress and difficult situations

Ability to work in line with protocols

Positive attitude towards feedback and self-development

We are one of the largest practices in the area with a current list size in excess of 15,000 patients, spread over 3 sites. We have an established clinical and administrative team and as a practice are committed to provide best possible care in a friendly and supportive environment. We are Nottingham University Teaching practice.

Job description

Job responsibilities

The purpose of the role is to:

Offer general assistance to the Practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone

Receive, assist and direct patients in accessing the appropriate service or healthcare professional in a courteous, efficient and effective way

Undertake a variety of administrative duties to assist in the smooth running of the busy reception.

Main duties will include:

  • Opening up/locking-up of Practice premises and maintaining security in accordance with Practice protocol
  • Maintaining and monitoring the Practice appointments system
  • Processing personal and telephone requests for appointments, visits and telephone consultations and ensuring callers are directed to the appropriate healthcare professional.
  • Providing signposting and care navigating services to our patient population ensuring patients are seen by most appropriate clinician.
  • Taking messages and passing on information
  • Filing and retrieving paperwork
  • Processing repeat prescriptions in accordance with Practice guidelines
  • Computer data entry/data allocation and collation; processing and recording information in accordance with Practice procedures
  • Initiating contact with and responding to requests from patients, other team member and associated healthcare agencies and providers
  • Clearing and re-stock consulting rooms as required

This post(s) will be based across all 3 sites and therefore driving license and access to own car is essential.

Previous GP reception or/and administration experience would be an advantage, however full training will be given to the right candidate.
