Job summary
Pulborough Medical Group (PMG) is situated in picturesque Rural Sussex in modern purpose built premises. We have a list size of 13,500 patients with 4 GP Partners and 9 salaried GPs. PMG is a training practice with 2 GP trainers and has a great nursing team of 7 nurses and 2 HCAs who are managed by our Nurse Partner.
This is a new role, which we believe will be valuable for our patients and team. The post holder will be able to develop this role over time in collaboration with the other clinicians.

Main duties of the job
We have a vacancy for a part-time Advanced Nurse Practitioner. This new role will support our GPs with the urgent minor illnesses, minor injuries and triaging of patients phoning in for 'on the day' appointments and advice from our clinicians.
We can be flexible with start times and could offer a term time contract. Ideally we are looking for 25 hours per week: 9.00am - 2.00pm (Monday - Friday).
About us
Pulborough Medical Group has over 50 staff members, and we all work together as a team. As we are in rural Sussex, we offer lots of community services, so that patients do not need to travel to hospitals for their outpatient appointments. Examples of these services are: audiology, physiotherapy, dermatology, vasectomy and women's health.
The Community Nurses and Health Visitors are based in our building as well as a large Kamson's pharmacy.

Job summary
This is a new role, designed to support the GPs with the urgent minor illness and minor injury workload. It will be an evolving role, which the post holder can develop in collaboration with the other clinicians at PMG.
The post holder will be a caring, compassionate and committed experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient including initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within general practice. The post holder will commit to demonstrating critical thinking in the safe clinical decision-making process. They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing high quality nursing care.
Key responsibilities
Clinical practice
Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs and proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition
Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan
Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice
Work with patients in order to support compliance with and adherence to prescribed treatments Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions
