As a Band 5 Social Prescribing Link Worker, youll empower adults, children, and families to overcome challenges, build confidence, and access the community-based support that truly makes a difference. Working closely with GPs, schools, local authorities, and grassroots organisations, youll connect people with the resources that help them thrive from social activities and mental health support to welfare advice and early-help services. Main duties of the job

  • Provide personalised 1:1 and group wellbeing support, helping people set goals and access community resources.

  • Support adults and young people, as well as families facing social, emotional, or practical challenges.

  • Build strong, collaborative relationships with primary care, schools, social care, and community organisations.

  • Use coaching, motivational interviewing, and strength-based approaches to empower individuals to make positive changes.

  • Keep accurate records, track progress, and contribute to monitoring and improving the service.

  • Develop and maintain detailed knowledge of local services and create effective referral pathways.

  • Engage with diverse and marginalised communities to ensure inclusive, culturally sensitive support.

  • Promote safeguarding, equality, and inclusion in all aspects of the role.

    Chapeltown Primary Care Network (PCN) brings together several local GP practices (Allerton and Westfield, St Martins and Woodhouse Medical Practice) to deliver coordinated, community-focused healthcare for the people of Chapeltown and surrounding areas. The network works to improve health outcomes, reduce inequalities, and provide more joined-up support by integrating primary care with wider health, wellbeing, and community services.

    The PCNs multidisciplinary team includes clinical pharmacists, pharmacy technicians, paramedics, an occupational therapist, health and wellbeing coaches, and social prescribing link workers. This team works alongside GPs, care coordinators, and local voluntary-sector partners to provide holistic, person-centred support. Together, they help patients manage health conditions, improve wellbeing, and access the services and community resources that best meet their needs.

    We are seeking a compassionate and motivated Band 5 Social Prescribing Link Worker to join our multidisciplinary team. This role is pivotal in supporting people with long-term health conditions, as well as children and families, to improve their wellbeing through non-clinical, community-based support.

    As a Link Worker, you will work in partnership with primary care professionals, local authorities, schools, and voluntary/community sector organisations to empower individuals to take greater control of their health and wellbeing. You will address the wider social determinants of health by connecting people with a range of services including community groups, parenting support, physical activity opportunities, mental health resources, welfare advice, and more.

    Key Responsibilities

    • Provide personalised 1:1 and group support to individuals referred to the service, helping them to identify their goals, develop a wellbeing plan, and connect with appropriate community resources.
    • Work with adults living with long-term physical and/or mental health conditions to improve self-management, resilience, and independence.
    • Support children and families, including those experiencing social isolation, low-level mental health issues, or socio-economic challenges, to access appropriate local services and improve wellbeing.
    • Collaborate with GPs, health visitors, school nurses, community paediatrics, social care, and local voluntary organisations to ensure a coordinated, holistic approach.
    • Use motivational interviewing, coaching, and strength-based approaches to empower service users to make positive changes in their lives.
