We are looking for a Nurse to join
our friendly and supportive Nursing Team -consisting of three Practice Nurses
(ranging from Staff Nurse to Senior Sister), 2 Health Care Assistants and a HCA
Apprentice, at our thriving, split-site, training practice in Central and North
Leeds.
The successful candidate will be committed to, and
responsible for, the delivery of high quality, patient-focused Practice Nursing
services for our diverse listing of 10,000 patients. The role will be in supporting
patients to be healthy, management of long term conditions, health prevention
and screening
Main duties of the job
The delivery of high quality, patient-focussed Practice Nursing services for our diverse listing of patients. The role will be in supporting patients to be healthy, management of long-term conditions, health prevention and screening as well as providing nursing assistance to the doctors and other members of the primary healthcare team.
You will be responsible for a
range of clinical areas including wound and ulcer care, immunisations,
venepuncture, ECGs and assist with minor surgery. The role will assess, plan,
develop, implement and evaluate programmes that promote health and well-being,
and prevent adverse effects that are detrimental to health. Implement and
evaluate individual treatment plans for patients with long-term conditions, and
identify and manage appropriately patients at risk of developing a long-term
condition. Prioritise health problems and intervene appropriately to assist the
patient in complex, urgent or challenging situations. Support patients to adopt
health promotion strategies that encourage good health, deliver opportunistic
health promotion at all times and support the principles of self-care.

b) Based on competencies support the team in the delivery of long-term condition management and take responsibility for certain clinical elements.
c) Assist in and perform a variety of nursing tasks related to patient care.
Such tasks may include:-
- Basic wound care / ulcer care
- Removal of sutures
- General patient health checks
- Learning Disability health checks
- Cardiovascular disease checks
- Urinalysis
- ECGs
- Peak flow readings
- Venepuncture
- Blood pressure measurements
- Ear syringing
- Routine immunisations
- Chaperoning and assisting patients where appropriate who are being examined by another clinician
- Assisting GPs with minor surgery and contraceptive implant fittings and removals
- Requesting basic pathology tests, for example urine culture, swabs
- Following agreed clinical protocols with referral to senior nurses or GPs as appropriate
Other tasks which may include:-
- Maintaining and cleaning equipment used by the Nurses and GPs
- Maintaining GP and Nurses rooms, stocking, and rotating items as required
- Chaperoning and assisting patients who are being examined by another clinician.
- Undertake housekeeping duties including general tidiness and cleanliness of nurses and treatment rooms
- Ordering and maintaining Health Promotion Literature
- Ordering of stock
- Participation in administrative systems in the Practice
- A duty to advise senior nurses of potential problems or errors within the range of assigned tasks
- Attend and participate in any Practice and Nurse Meetings when required.
