Job summary

An exciting opportunity for a Clinical Pharmacist to join South West PCN here in Milton Keynes. To be part of a dynamic team working towards optimal patient care.

Main duties of the job

The successful candidate will need to understand the Pharmacist role in Primary Care within the PCN setting.

Will provide specialist pharmaceutical advice, perform face to face medication reviews and ensure effective Medicines Optimisation in all settings.

You will also assist in safety and quality of medicines as well as conducting annual audits.

We are South West Primary Care Network located in Milton Keynes.

A group of 4 GP practices comprising of Westcroft Health Centre, Westfield Road Surgery, Parkside Medical Centre and Bedford Street Surgery.

Job responsibilities

The post holder will work within their clinical competencies to provide specialist pharmaceutical support to all practices of the PCN including working with members of the wider healthcare team to improve care and support safe and effective prescribing. Moving between practices, the role will have a patient facing element either face to face or telephone - including chronic disease management, repeat prescription management and structured medication reviews. This will also include the management and review of patients with polypharmacy. The post holder will need to be proactively involved in quality improvement and audit as well as managing some aspects of the Quality and Outcomes Framework. There will also be the requirement for the post holder to contribute to the provision of out of hours/extended hours/improved access services. The post holder will be supported to help develop the role and work to improve the care and outcomes for the patients across South West PCN. To play a pivotal role in the delivery of high quality primary health care within South West PCN.

Qualifications

Essential

  • Diploma in Clinical Pharmacy.
  • Independent Prescriber or willing to undertake course.

Desirable

  • Qualification in respiratory medicine desirable but not essential, depending on experience, e.g. COPD or Asthma Diploma or other relevant chronic disease management.

Experience

Essential

  • Experience of working at a high level of clinical complexity.
  • Broad based knowledge of both chronic and acute disease to enable full medication reviews and the handling of medication queries.
  • Familiarity with QOF and Prescribing Incentive Schemes and experience of different approaches to ensuring targets are met.
