Job summary
We are recruiting for an experienced Practice Nurse to join our Nursing Team and co-ordinate the programme of care for our diabetic patients.
Our Practice Nurses are supported by our Lead Nurse and deliver nursing services within the boundaries of their scope of practice to the practice population. The Practice Nurses are responsible for several clinical areas such as health promotion, chronic disease management, health prevention, as well as supporting the management team in the reviewing of clinical policy and procedure. They work collaboratively with our teams to meet the needs of patients and provide excellent and safe patient care. Our nursing team work across 3 of our patient-facing sites within Waterlooville.
We are looking for a motivated team player who is highly organised, enthusiastic and patient-focused with a can-do attitude and friendly personality. Candidates must have evidence of Diabetes training (Diploma, PitStop or other accredited course)
We can consider candidates seeking 30 - 37.5 hours per week between Monday- Friday, with a working pattern to be agreed. Extended hours will form part of the working pattern which may include Saturday shifts on a rotational basis.

Main duties of the job
To work as part of our nursing team to provide a diverse and comprehensive range of nursing services and care to our patient population.
To focus on
supporting patients living with diabetes to independently manage their condition,
including the provision of diabetes clinics and referrals for
patients.To support colleagues with complex diabetes queries and
help to develop the diabetes strategy.
To support the following clinics/specialities and provide hands on, autonomous patient care:
- Wound Care
- Leg Ulcer Clinics
- Long Term Condition Management (Hypertension, CHD, CKD, Diabetes, COPD/Asthma)
- Cervical Cytology
- Childhood and Adult Immunisations
- Travel Health

- Work within clinical boundaries, demonstrating safe
and competent clinical decision-making, delivering high quality clinical care
and maintaining patient confidentiality at all times
- Assist and
perform tasks related to patient care as directed by Lead Nurse and GPs
following agreed clinical protocols
- Deliver
evidence-based care and structured education to people with diabetes, their
families and other health colleagues in a primary care setting. This includes
participating in joint clinics and domiciliary visits for patients with
diabetes including those living in Care Homes (who have complex needs) whilst
educating primary care staff to enhance their skills to provide an excellent
diabetes service
- Participate
in local Diabetes Steering Groups, particularly looking at clinical standards,
guidelines, pathways and improving outcome measures
- Support and participate
the internal diabetes meetings, updating colleagues on new NICE and local
guidance and sharing prescribing knowledge
- To provide
essential treatment duties such as BP checks, NHS Healthchecks, prescribed
injections (e.g. B12 and Denosumab) BMI and lifestyle advice
- Provide
holistic assessment of wounds and implement wound care including complex wound
management in line with current evidence-based guidelines such as National
Wound Care Strategy (Dopplers/Compression)within own level of competency
- Support
NAs/HCAs carrying out wound care by maintaining clear care plans
- Perform a
holistic assessment of patients attending for relevant investigatory procedures
such as cervical cytology, ECG, following training and in line with local
guidelines
