Job summary

The post holder will be an experienced Primary Care Nurse providing high quality care for patients, including chronic disease management, smears, immunisations, health reviews, wound care, minor illness between the hours of 8am and 6.30pm across our 3 surgery sites.

The nurse will be responsible for ensuring clinical protocols, procedures are reviewed annually and carrying out relevant audits, such as infection control.

The individual will work within the NMC Code of Conduct to agreed practice standards and protocols.

Contribute to the practice achieving its GMS contract obligations including quality, organisational and access targets. Working within the requirements of the CQC.

Main duties of the job

Deliver specialist primary care service, including chronic disease management including for all areas of QOF, in line with agreed protocols and reference to the General Practitioner, where necessary.

Responsible for managing clinics, carrying out associated tasks including: Cytology, wound care, removal of sutures, peak flow readings, venepuncture, management of long-term conditions. Administering child and adult immunisations and vaccinations in accordance with national and local programmes. Chaperoning patients being examined by another clinician, urine testing, dietary advice, anaphylaxis and CPR.

Job description

Job responsibilities

Deliver specialist primary care service, including chronic disease management including for all areas of QOF, in line with agreed protocols and reference to the General Practitioner, where necessary.

Make professionally autonomous decisions, for which they are accountable and provide safe, evidence-based patient care.

Responsible for managing clinics, carrying out associated tasks including: Cytology, wound care, removal of sutures, peak flow readings, venepuncture, management of long-term conditions. Administering child and adult immunisations and vaccinations in accordance with national and local programmes. Chaperoning patients being examined by another clinician, urine testing, dietary advice, anaphylaxis and CPR.

Make appropriate referrals to other members of primary, community and secondary care.

Promote health and wellbeing, giving general education advice on diet, contraception, smoking, chronic disease management, exercise etc

Ensure safe storage, rotation and disposal of vaccines and drugs, including stock control.

Where the post holder is an independent prescriber: to ensure safe, effective and appropriate medication as defined by current legislative framework.

Maintain accurate records according to NMC rules.

Ensure all data protection requirements are met when gathering, recording and storing patient data.

Liaise and maintain good working relationships with all members of the practice team.

Be flexible and provide clinical cover for sessions in extended hours and contribute towards meeting the extended access requirements.Professional Role

Attend and contribute to relevant internal and external meetings as required including practice clinical meetings, nurse team meetings and clinical supervision meetings.

Promote evidence-based practice through use of the latest research based guidelines.

Work collaboratively with colleagues both internally and externally.

Partake in clinical supervision for own development purposes to develop the nursing staff.

Demonstrate due regard for the safeguarding of children and vulnerable adults.

Ensure that health updates, alerts, policies, and relevant newsletters are accessed and acted on, where necessary.
