Job summary

St Johns Surgery is pleased to offer an exciting opportunity for an additional Clinical Pharmacist to work with us under the PCN Additional Roles Scheme.

Main duties of the job

The successful applicant will support the Surgery in meeting its obligations under the PCN Network Enhanced Service and will work alongside the pre-existing pharmacists at the Surgery and other pharmacists at the 9 practices within the Bromsgrove and District Network. The role will be an integral part of the day-to-day Surgery functioning. There will be opportunities for patient-facing roles to clinically assess and treat patients using expert knowledge of medicines, alongside scope for specialist clinics and prescribing.

Job description

Job responsibilities

The successful applicant will support the Surgery in meeting its obligations under the PCN Network Enhanced Service and will work alongside the pre-existing team of pharmacists at the Surgery and other pharmacists at the 9 practices within the Network across the Bromsgrove District. The role will be an integral part of the day-to-day Surgery functioning as well as the wider PCN with opportunities for patient facing roles to clinically assess and treat patients using their expert knowledge of medicines alongside scope for specialist clinics and prescribing. The responsibilities will include conducting Structured Medications Review and Medicines Optimisation to tackle polypharmacy, audit and review of medicines through NHS-led programmes such as Low Priority Prescribing and CCG driven cost saving switching. There will also be a key role in supporting the Enhanced Health in Care Homes agenda for the 90 residents currently registered with the practice with a view to the benefits of both new patient and regular clinical pharmacist led medicines reviews. For an appropriate candidate, there will be potential scope for professional development in evolving skills in independent prescribing and through specialist clinics including asthma/COPD, family planning, diabetes and community based anticoagulant monitoring.
