Job summary
The post holder is an experienced nurse who, acting within their professional boundaries, will provide care for the
presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They
will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for
patients within the general practice. The post holder will demonstrate critical thinking in the clinical decision-making
process. They will work collaboratively with the general practice team to meet the needs of patients, supporting the
delivery of policy and procedures, and providing nurse leadership as required. In order to work at this level NMC
requirements for advanced practice must be met.
Main duties of the job
- You must be able to conduct a comprehensive physical health assessment of patients with complex multiple healthcare needs.
- Interpret results of multiple different assessments and investigations to make a diagnosis, and plan to deliver care.
- Competently make ethical, evidenced decisions and interventions when faced with complexity and assess the risk associated with these decisions.
- Prescribe and work with individuals to manage their medicines, and to be able to refer where necessary.
- Work independently but also as part of a multi-disciplinary team
- Plan and provide skilled and competent care to meet a patient's health and social care needs involving or referring on to other members of the healthcare team as appropriate.

About us
We are dynamic, forward looking Practice with 3 Partners, 6 Salaried GPs, Advance Nurse Practitioner, 2 Practice Nurses, HCA, in house Pharmacist supported by our Receptionists and Administrative Team. We are a Practice that performs well in QOF and other national and local Agreed Services. We currently hold a Good rating with the CQC. The Practice is currently looking to recruit a friendly, enthusiastic and hard-working Advanced nurse practitioner to join our team. You will work with the practice team to meet the needs of patients, nurses and GPs following policies.
Applicants must be educated to the appropriate level, have knowledge of GP Practice procedures and good IT skills. Previous experience in a General Practice and with EMIS is preferred but not essential.
