This is an exciting opportunity to join the existing team at New Hayesbank Surgery. We are a member of Mid Kent PCN comprising of 7 practices. This role will be collaborating with colleagues but main workplace will be New Hayesbank Surgery.

The successful candidate wil work as part of a mult-disciplinary team in a patient facing role, taking responsibility for areas o chronic disease management and clinical medication reviews.

The successful candidate must be registered with the General Pharmaceutical Council (GPhC) and meet the specific training requirements as specified by the GPhC criteria to register as a Pharmacy Technician.

The post is funded via the Additional Roles and Reimbursement scheme.

Main duties of the job

Main duties of the job

Undertake patient facing and patient supporting roles to ensure effective medicines use, through shared decision making conversations with patients.

Carry out medicines optimisation tasks, supporting medication reviews and medication reconciliation.

Take a central role in the clinical aspects of shared care protocols liaising with relevant teams for support.

Assist in the delivery of medicines optimisation and management in incentive schemes and patient safety audits.

We are registered with the CQC and proud of our "outstanding" rating. We have a large multi-disciplinary team who support the GP partners in providing the best possible care for our patients. We are also the Lead Practice for our PCN and one of our GP Partners is Clinical Director.

We are a training practice and also run a 7 day a week ophthalmology service which includes cataracts, wet AMD and general ophthalmology. We also have a number of in-house clinics.

Job responsibilities

To work with practice based pharmacy technician and prescribing clerks to support the Practice Based Clinical Pharmacist to ensure the delivery of safe, effective and efficient systems for repeat prescribing efficiency, medicines optimisation, Medication enquiries Triage and maximising patient outcomes.

The role will also include training and development of practice based prescribing co-ordinators and administrators to support improved co-ordination and effective pharmacy administration within general practice, especially with relation to repeat prescribing systems and processes.

Job responsibilities

To undertake medicines reconciliation in accordance with practice protocols and to update patients medical records / prescribing accordingly.

To support the achievement of the practices prescribing Quality and Outcomes Framework targets.

Working with the Clinical Pharmacist and Prescribing Co-ordinators to contribute to the review and development of repeat prescribing protocols / Standard Operating Procedures to improve the efficiency and effectiveness of repeat prescribing systems.

To provide advice to GPs, staff and patients in changes to prescribing to support the improvement of prescribing safety, quality and cost effectiveness.

To participate in medication audits.

To assist in the appropriate monitoring and management of prescribing budgets and to liaise with the medicines management team at the Kent and Medway CCG.

To prepare evidence based resources and information to support the implementation of rational cost-effective prescribing.

To support the Practice Based Clinical Pharmacist to provide medication reviews, patient information leaflets, medicine awareness; and to undertake community visits as appropriate.

To take a proactive approach to the safe handling of prescriptions, including assessment of risk and making recommendations for improvement.

To respond to medication queries from patients and staff in a professional manner referring to the appropriate GP in accordance with practice protocols.

To advise on the sourcing and safe management of medicines as appropriate.

To assist in the submission of claims for personally administered drugs as required.

Be responsible for the organisation, planning and of own workload to meet set deadlines.
