Job summary
The post holder will lead the medications optimisation work for the S&W Herefordshire PCN. This role will take ownership for the PCNs successful achievement of all contractual requirements and will report on progress against these targets to the PCN Board. In addition to working within the scope of their clinical competencies in direct patient care, the role will act to coordinate the pharmacists and pharmacist technicians across the network to ensure that all practice-based roles have the information and skills required to successfully undertake the required medications work within the PCN.
Main duties of the job
The role will be varied and will work across a range of different activities and clinical conditions, including:
Oversight of the medications elements of the PCNs national Direct Enhanced Service (DES) contract and any local medications contracts
Undertaking structured medication reviews across designated cohorts of patients. These will include but not be limited to care home residents, patients on 10+ medications, patients on potentially addictive medication, patients over 85-years and patients at risk of drug interaction.
Management of long-term conditions, with a particular focus on cardiovascular disease management
Reviewing systems for safer prescribing
Contributing to repeat prescription authorisations and reauthorisation
Actioning acute prescription requests
Addressing both the public health and social care needs of patients
Undertaking clinical audit
The post holder will be supported by a GP clinical mentor at Alton Street Surgery and the Clinical Director of the PCN. They will also engage with pharmacy colleagues across Herefordshire for peer support.
The Pharmacist will ensure that PCN Practices integrate with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare, and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.

South & West Herefordshire PCN is made up of six practices centred around the market town of Ross-on-Wye: Alton Street Surgery, Pendeen Surgery, Fownhope Surgery, Much Birch Surgery, Golden Valley Practice and The Surgery Kingstone. The network covers a wide geographic area offering traditional General Practice support to our patients centred around the communities they live in. The position will be hosted by Alton Street Surgery as the employer.
