Main duties of the job
To provide a
high-quality, cost-effective service to the patients of the Practice; focusing
on frail and older people, and those with low-level mental health concerns. You will also work with and triage other patients outside of frail and elderly.
You will be
working as part of a multi-disciplinary team which include, GP Partners, GPs,
Advanced Clinical Practitioners, Nurse Practitioners, Paramedic Practitioners,
Clinical Pharmacists, Physiotherapists, Practice Nurses and Healthcare
Assistants.
You will be
responsible for face to face and telephone consultations with patients
presenting with a wide range of minor illness, urgent and same day requests for
treatment and chronic disease management, providing care for the presenting
patient from initial history taking, clinical assessment, diagnosis, treatment
and evaluation of care.
You will need
to demonstrate safe, clinical decision-making and expert care, including
assessment and diagnostic skills. You will need to prioritise and triage the
needs of patients accordingly making any necessary referrals for investigations
in the appropriate manner.
A large
percentage of the role will be involved the care of frail and elderly patients
including the responsibility of patients within care and nursing homes. This will include weekly ward rounds.
Part
of your responsibilities will include visiting patients in their home to assess
their medical condition and frailty review. You may also be involved in triage
of patients.
