Job summary

We are seeking a compassionate and organised individual to join our team as a Care Navigator.

As a Care Navigator at Ivy Medical Group, you will play a vital role in supporting patients by helping them access the right care and services. You will work closely with healthcare professionals to ensure a seamless and coordinated care experience for our patients.

Main duties of the job

Key Responsibilities:

  • Patient Care - Signposting: Process and effectively signpost patients to the appropriate healthcare professional, depending on the presenting condition
  • Patient Support: Act as the main point of contact for patients using our online services, guiding them through their healthcare journey and assisting them in navigating the services available to them ensuring the patient receives the most appropriate care that they need.

  • Practice Support: Support the practice clinical team with coordinating the outcomes for patients.
  • Administrative Duties: Handle patient records, schedule appointments, and support other administrative tasks related to patient care. Ensure that all interactions are logged accurately in the system.

Job description

Job responsibilities

The following are the core responsibilities of the care navigator. There may be, on occasion, a requirement to carry out other tasks; this will be dependent on factors such as workload and staffing levels.

a. Process and effectively signpost patients to the appropriate healthcare professional, depending on the presenting condition

b. Process patient requests for appointments

c. Process all online requests

d. Initiate contact with and respond to requests from patients, team members and external agencies

e. Input data into the patients healthcare records as necessary, enter read/SNOMED CT code data

f. Direct requests for information, e.g., SAR, insurance/solicitors letters and DVLA forms, to the administrative team

g. Manage all queries as necessary in an efficient manner

h. Carry out system searches as requested

i. Maintain a clean, tidy, effective working area at all times

j. Support all clinical staff with general tasks as requested

k. Undertake all mandatory training and induction programmes

l. Attend a formal appraisal with their manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed

Contribute to public health campaigns (e.g., flu clinics) through advice or direct care

In addition to the primary responsibilities, the care navigator may be requested to:

a. Participate in practice audit as directed by the audit lead

b. Support administrative staff, providing cover during staff absences

c. Action incoming emails when necessary

d. Scan patient-related documentation and attach scanned documents to patients healthcare records

e. Complete opening and closing procedures in accordance with the duty rota

f. As required, support in the process of repeat prescriptions, ensuring that they are processed accurately and efficiently

  • Educated to GCSE Mathematics & English (C or above) or equivalent

Desirable

  • Active signposting or Care Navigator training qualification.
  • Healthcare qualification (level 2) or working towards gaining equivalent level.
  • Customer service qualification (NVQ) or equivalent.

Experience

  • Experience of working in a primary care environment.
  • Experience of working with the general public.
  • Experience of working in a healthcare setting.
  • Experience of administrative duties.
  • Excellent communication skills (written and oral).
  • Strong IT skills.
