Job summary
The Birmingham Solihull Training Hub are looking to recruit a Personalised Care Role Facilitator to support the
Birmingham Solihull Training Hub with the development of the personalised care
workforce which includes Social Prescribing Link Workers, Care Coordinators,
Health and Wellbeing Coaches, across GP practices in the Birmingham Solihull
Integrated Care System (BSol ICS) footprint. The post
holder will act as a Facilitator to support the development of this workforce
within primary care.
The individual should have experience working as either a Social
Prescribing Link Worker, Health and Wellbeing Coach or Care Co-ordinator and
should have working knowledge of the Personalised Care roles and how they link
together.
The post
holder will have good oral and written communication skills and
exceptional organisational and administrative skills.
Main duties of the job
Key responsibilities include, but not limited
to:
- To participate in
recruitment and training of personalised care roles, which includes Social
Prescribing Link Workers, Care Co-ordinators and Health and Wellbeing
Coaches
- Build working
relationships with lead employers, trainers and any key stakeholders to
facilitate the development of these roles
- Promote the roles across
the BSol footprint, in accordance with local and national guidelines
- To offer guidance, support
and updates to personalised care roles
and other staff working in general practice
- Identify areas for
workforce development with regards to these roles
- Train and support
these professionals to perform
optimally in their roles across the BSol footprint
About us
The successful candidate will need to have a Level 3 qualification in a relevant field (Health & Social Care, Health Trainer, Health Improvement, Community Family Worker, Psychology) or be working towards/willing to work towards. In addition to this they should be trained in Motivational Coaching and Behaviour Change and show demonstrable commitment to professional and personal development.

This role will be
required for 22.5 hours per week. This
is on a 12 month fixed term contract with the potential to be extended on
successful funding being obtained.

recruitment and development of the roles with key stakeholders including primary care employers, Primary Care Networks
(PCN), BSol CCG, BSol ICS Health Education England (HEE), Personalised Care
Institute (PCI) and NHS England/Improvement (NHEI). The post is an
interactive role, developed in response to the demands of primary care, new
emerging roles for paramedics in primary care and the latest GP Contract Additional
Roles Programme.
Reporting to the Training Hub
Operations Manager, the Personalised Care Role Facilitator will maintain responsibility
to support staff working in the personalised care roles and projects in
relation to workforce development within the BSol footprint. Demonstrating a
commitment to high standards of service delivery and training, you will be a
learning facilitator with demonstrable enthusiasm for paramedic roles in
primary care. The role may also require
the post holder to support the development and recruitment of other Allied
Health Professional (AHP) when required.
Key responsibilities
The post holder will be employed by
South Doc Services working within the Birmingham Solihull Training Hub, to
deliver duties and responsibilities as
listed:
- To participate in
recruitment and training of personalised care roles, which includes Social
Prescribing Link Workers, Care Co-ordinators and Health and Wellbeing
Coaches
- Build working
relationships with lead employers, trainers and any key stakeholders to
facilitate the development of these roles
- Promote the roles
across the BSol footprint, in accordance with local and national
guidelines
- To offer guidance,
support and updates to personalised care roles and other staff working in general
practice
- Identify areas for
workforce development with regards to these roles
- Train and support these
professionals to perform optimally
in their roles across the BSol footprint

personalised care training amongst all health professions including, but
not limited to, Pharmacists, Physiotherapists etc

members and healthcare professionals on matters beyond level of competence
- Support the
identification and development of learning opportunities for the personalised
care roles and contribute to future curriculum development with training
providers where applicable
- Respond to
requests for advice and support from other team members
- Contribute to
patient care and reduce hospital admissions by assisting with the design
of services for the personalised care roles to support and implement
- Adhere to organisational,
local and national guidelines
- Collect data for
audit purposes
- Attend meetings as
required
- Undertake
additional training where necessary to provide improved or new services
- Participate in the
administrative and professional responsibilities of the service team
