About us
Carn to Coast Health Centres is an 11-Partner
Cornish GP Practice serving over 30,000 patients. We have a main surgery in
Pool and branch surgeries in Illogan, Camborne and St Day.
Our Practice has historically been at the forefront
in establishing a highly skill-mixed approach which includes includes paramedics, mental health staff, clinical pharmacists and technicians,
care co-ordination and physiotherapy, all supported by a substantial
administrative team, whose role includes processing clinical correspondence and
workflow (through Docman).
Our catchment is broad with urban, rural and
semi-rural elements including areas of deprivation which we seek to engage as
part of a programme of population health management so that continuity of care
is assured where it is most needed.
Notwithstanding the relatively large size of the
practice, our ethos remains friendly, supportive and very informal in style. We
look to everyone to make a contribution and be heard and are always looking at
ways to improve and provide scope for advancement at all levels. (Our
partnership already includes three partners who are not doctors.) We have
regular team meetings and have a strong emphasis on teamwork across the
organisation, regardless of role and seniority. We are confident that the innovative model we have
developed and continue to refine is well suited to meet those challenges going
forward and to embrace the increasing role of primary care networks within the
system.
