About the Role:
An exciting opportunity has arisen for a highly motivated and organised individual to join our friendly and forward-thinking team at Falmouth and Penryn PCN as a Pharmacy Designated Patient Care Coordinator.
We have a thriving clinical pharmacy team comprising 7 pharmacists and 3 pharmacy technicians who work collaboratively across our practices to deliver high-quality, patient-centred care. As we continue to develop our pharmacy services, we are now looking to appoint a dedicated Patient Care Coordinator to provide essential administrative and patient-facing support to our pharmacy team.
This is a newly created role that will play a key part in improving patient access to pharmacy-led services, enhancing the safety and efficiency of medicines management, and supporting the day-to-day operations of the pharmacy team within general practice.
Main duties of the job
Initially, we are hiring for this position on a 12 month fixed-term contract. However, there is potential to expand the contract beyond this depending on the success of the role and the position of the team at the end of the fixed-term.
Key Responsibilities:
Act as a key point of contact for patients with medicines-related queries, ensuring excellent communication and compassionate service.
Support the smooth running of medication reviews, repeat prescription processes, and medication safety audits.
Liaise between patients, community pharmacies, and the practice team to ensure continuity of care.
Assist in managing long-term condition recalls, medication monitoring schedules, and proactive patient follow-up.
Maintain accurate records and use clinical systems (such as EMIS/SystmOne) to update and retrieve patient information.
Support delivery of public health campaigns related to medication use, prevention, and health promotion.
About us
The Falmouth and Penryn PCN was formed in 2019. It brought together 4 local GP surgeries across Falmouth and Penryn, serving approx. 48,000 patients. With the combined workforce, skills and knowledge, we are in a unique position to assist our patients across the county. We provide care for an interesting and diverse population, ranging from students and young families to the elderly. The four GP practices are geographically close to one another and have a long history of working together.

There is a wide socio-economic demographic served within this area which provide an interesting and varied case mix. Act as the first point of contact for patients, carers, and healthcare professionals regarding medicines-related queries.

Support the coordination and scheduling of medication reviews, polypharmacy reviews, and structured medication reviews led by pharmacists.

Assist with managing medication-related recalls and monitoring for long-term conditions (e.g., blood tests, blood pressure monitoring, medication safety checks).

Process incoming and outgoing communications related to medication queries, prescription requests, and patient follow-up.
Assist in the collection, reporting, and analysis of data to support medication safety audits, service development, and Quality and Outcomes Framework (QOF) indicators.
Maintain electronic patient records in accordance with data protection legislation, GDPR, and practice policies.
Team Working & Communication:
