Job summary

We have an exciting opportunity for an Advanced Nurse Practitioner to join the Tone Valley Primary Care Network (PCN) Proactive Care (PAC) team.

This senior clinical role integrates responsibilities from both community and care home settings. The postholder will lead proactive, personalised care for patients with complex needs, frailty, or health inequalities, both in care homes and the wider community. They will manage a caseload, coordinate multidisciplinary input, and deliver holistic assessments, personalised care plans, and treatment escalation plans (TEPs).

Main duties of the job

The role supports the Enhanced Health in Care Homes (EHCH) framework and NHS Englands Proactive Model of Care, aiming to reduce unplanned care, promote independence, and improve quality of life.

They have been assessed as competent in practice using their expert clinical knowledge and skills. They have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis and treatment of patients.

The ANP provides an opportunity for patients to avoid the need to be referred to a GP, thereby enabling them to receive timely care and negate unnecessary delay in receiving treatment.

ANPs in primary care can develop close, long-term relationships with their patients and work in partnership with them to achieve optimum health. They are autonomous in making decisions based on assessment, diagnosis and the interpretation of test results. They can independently prescribe appropriate medication, evaluate or refer to other specialists if necessary.

About us

Tone Valley PCN consists of 5 like-minded Somerset practices; Lyngford Park Surgery, Taunton Vale Healthcare, Creech Medical Centre, Warwick House Medical Centre, North Curry Health Centre and are responsible for the care of around 36,500 patients.

Travelling across all five Practices and sites is a requirement for this role.

Salary: Inductive Banding under ARRs:

- Band 8a (AfC): Applicable where the postholder has achieved and holds a recognised Digital Badge

- Band 7 (AfC): Applicable where the postholder is actively undertaking the Advanced Clinical Practice (ACP) apprenticeship pathway

Interviews: Expected to take place on Thursday 11 December at Creech Medical Centre

Key responsibilities:

a. Provide holistic support in the clinical governance spectrum assuming a senior role in its compliance

b. Support the IPC lead for the proactive care team

c. Monitor and ensure the safe storage, rotation and disposal of medications

d. Participate in local initiatives to enhance service delivery and patient care

e. Support and participate in shared learning within the organisation

f. Develop an area of specialist interest, taking the lead within the organisation

g. Continually review clinical practices, responding to national policies and initiatives where appropriate

h. Drive the development of nursing services within the organisation, liaising with external agencies and professional organisations as required

i. Develop practice administrative and clinical protocols in line with the needs of the patient and current legislation

Clinical Care and Patient Services:

a. Manage a caseload of patients identified through MDTs and risk stratification tools (e.g., EMIS, Ardens, Brave AI)

b. Involvement in weekly Proactive Care Triage of referrals and cohorts identified through risk stratification, with the teams and with support of cross system IT systems

c. Conduct holistic assessments and develop Personalised Care and Support Plans (PCSPs) and TEPs

d. Deliver care home rounds and coordinate care with Lead GPs and the Nurse Practitioner in the EHCH Team

e. Support structured medication reviews with pharmacists support

f. Promote self-care and support patients to access community services (e.g., village agents, social prescribing)

g. Promote proactive care within care homes by educating care staff and residents

h. Support completion of Long-Term Condition checks, QOF, and Vaccination programs in housebound people and those struggling to engage in the GP system

Multidisciplinary Working:

a. Attend and contribute to weekly MDTs for care homes and community

b. Provide background information and updates on patients discussed

c. Liaise with internal and external stakeholders including social care, voluntary services, and secondary care

d. Implement actions from MDTs and monitor outcomes

e. Support sharing of information across team by use of shared IT platforms available

f. Be involved in Multi Agency Risk Assessment Meetings (MARMs) if required

Leadership and Supervision:

a. Provide clinical leadership and supervision to PAC Team

b. Support training and development of care home staff

c. Lead Quality Improvement (QI) projects and contribute to service development

d. Identify gaps in service delivery

Communication and Relationships:

a. Communicate effectively with patients, carers, and professionals

b. Share good news stories and learning across the PCN

c. Escalate safeguarding concerns appropriately through the safeguarding policy for Tone Valley

d. Maintain confidentiality and adhere to GDPR and information governance policies

e. Work closely with the fellow ANP in the TV Proactive Care Team

Planning and Organisation:

a. Prioritise workload and respond flexibly to service demands

b. Support reporting and analytics for PCN DES and EHCH specifications to Somerset ICB

c. Maintain accurate clinical records and use appropriate coding (e.g., SNOMED)

Policy, Research and Development:

a. Work within and contribute to the development of PCN policies and protocols

b. Participate in audits, evaluations, and clinical governance activities

c. Promote sustainability and equality in service delivery

Other Key Responsibilities:

a. Implement and evaluate individual specialised treatment plans for chronic disease patients

b. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health

c. Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF

d. Be aware of duties and responsibilities regarding current legislation and adhere to practice policies and procedures on Safeguarding Adults and Safeguarding Children

e. Request and collect pathological specimens. Upon receipt, to process and interpret pathology and other test results as required, according to competency

f. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

g. Maintain accurate clinical records in conjunction with extant legislation ensuring that SNOMED CT codes are used effectively

h. Maintain chronic disease registers and implement and embed an effective call/recall system

i. Support in the provision of vaccination programmes for both adults

j. Be a non-medical prescriber adhering to guidance

k. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required

  • Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet.
