Job summary
The main purpose of this role is to provide administration to the PCT Nursing team who provide proactive clinical support to vulnerable patients and those with complex physical or mental health needs who are at high risk of future hospital admission within the Winchester and Andover Primary Care areas. The aim is to improve their quality of life and healthcare experience and therefore reduce the risk of emergency hospital admission.
This role is on a fixed term contract (Maternity Cover) for a period of 12 months. Hours needed between 30 to 37.5 hours per week and might be worked over 4 or 5 days, depending on hours you are contracted for.
Main duties of the job
This role will include:
Managing and triaging incoming referrals into the Proactive Care Team.
Booking and planning clinical appointments for home visits.
Booking appointments onto EMIS (Clinical System) and updating clinical notes.
Preparing documentation for Nurse Appointments to include blood forms, assessment template, patient details, key safe numbers, and surveys (the list is not exhaustive).
Completing onward community referrals as required.
Provide advice and signposting to service users, carers, and professionals.
Co-ordinate clinical attendance at the Practice White Board Meetings.
Support the clinical team with administrative tasks, e.g., meeting minutes.
Complete audits as required.
Ensure that allocated patients can access services available in the community.
Review all elderly / complex patients post discharge notifications to identify those requiring a community follow up.
Ensure clinical team adhere to the Lone worker policy.
