As a Care Co-ordinator, you will be directly involved in supporting the Proactive Care Nursing Team to facilitate and shape self-care for patients with long term conditions in order to help keep them as well as possible in their own homes.

Main duties of the job

The main purpose of this role is to provide administration to the PCT Nursing team who provide proactive clinical support to vulnerable patients and those with complex physical or mental health needs who are considered to be at high risk of future hospital admission within the Winchester and Andover Primary Care areas. The aim is to improve their quality of life and healthcare experience and therefore reduce the risk of emergency hospital admission.
