Job summary

This is an exciting new role within Rosebank Health Primary Care Network. This role provides a great opportunity to join a committed team in providing additional care and services across PCN patient populations, including the provision of support and enhanced care to patients in residential and nursing home settings.

Main duties of the job

The aim of the Care Home Coordinator role is to work closely with the Primary Care Network and their associated residential, nursing and learning disability care homes. The Care Home Coordinator will ensure heath and care planning for our patients is timely, efficient, and patient-centred. You will support the provision of continuity of care and act as a point of contact for families, residents, and professionals.

The ideal candidate will have experience working in Primary Care and possess excellent communication and organisation skills, with a demonstrable passion for enhancing patient experience and outcomes.

About us

Rosebank Health PCN, includes Rosebank Surgery, Severnvale Surgery, Bartongate Surgery and Kingsway Health Centre. The successful candidate maybe required to work across all four sites.

The PCN is forward thinking and innovative, prioritising excellent patient care. The PCN has a friendly multidisciplinary team which includes: GP's, Nurses, Paramedics, Pharmacists, Physiotherapist and Social Prescribers.

Location: Working across 4 surgeries and their partnered care homes

Job summary:

The Care Home Coordinator will work as part of the primary healthcare team providing support and organisation to the surgeries clinical team and care home staff. Their role is to promote dignity, respect and ensure the highest quality of care is available for patients in residential, nursing and learning disability care homes. They will work collaboratively with the general practice team to help support the delivery of the Primary Care Networks care home policies and procedures.

Place of employment:

The main places of employment are to be Rosebank Surgery, 153b Stroud Road, Gloucester, GL1 5JQ, Severnvale Surgery, St James, Quedgeley, Gloucester, GL2 4WD, Kingsway Health Centre, Rudloe Drive, Kingsway, GL2 2FY, Bartongate Surgery, 115 Barton Street, Gloucester, GL1 4HR and any other premises in Gloucester that the Practice may operate from.

Key responsibilities:

  • Support the organisation, coordination and delivery of MDTs within the PCN;
  • Attend the MDTs and be the key person for ensuring attendance from nominated representatives or their deputies as defined for each home;
  • Ensuring MDT actions are recorded appropriately and followed-up within the defined timescales agreed at the MDT meetings;
  • Work with key PCN colleagues and Care Home leads to proactively identify residents for the MDT meeting, such as population health intelligence, clinical judgement and care home insight;
  • Support the registration of new care home patients, by checking all details have been provided and collected as required.
  • Ensure all patients registered within a care home are coded appropriately;
  • Provide dedicated support to residents and their carers who are having multiple simultaneous interactions with different health, care, and voluntary sector services, ensuring that care and support is coordinated and consistent;
  • Improve the continuity of care by acting as a point of contact for residents, families and professionals who visit the care home, such as MDT members and in-reach specialists;
  • Work with the GPs and other primary care professionals within the PCN to identify and manage a caseload of patients and where required and as appropriate, refer people back to other health professionals within the PCN;
  • Identify patients who havent been seen by a GP within the past 28 days and organising for them to be seen;
  • Be a part of the ongoing vaccination programme for all care homes;
  • Raise awareness within the PCN to shared-decision making and decision support tools;
  • Raise awareness of how to identify patients who may benefit from shared decision making and support PCN staff and patients to be more prepared to shared decision-making conversations;
  • Holistically bring together all of a persons identified care and support needs and explore options to meet these within a single personalised care and support plan (PCSP), in line with PCSP best practice, based on what matters to the person;
  • Coordinate annual reviews for patients with Serious Mental Illness, Dementia and Learning Disabilities;
  • Coordination of patients (asylum seekers) registration and care whilst staying at a local hotel;
