The role would include 2 days a week based in Shields Health Hub as part of the North Shields Enhanced Access Service.
We have an established team of 3 ANPs at Collingwood Health group already who would be excited to welcome you to their group

As a successful candidate you will work closely with the whole team, providing high quality acute, chronic and opportunistic health care as well as with practitioners from other organisations in the the 'hub' team.
You will develop close, long-term relationships with your patients and work in partnership with them to strive for optimum health.
You will work independently but alongside the extended clinical team offering and receiving mutual support.
Main duties of the job
You will act as an autonomous practitioner working independently and in conjunction with other health care professionals to assess, diagnose and treat the conditions of patients attending within primary care initiating direct referrals as appropriate.
You will provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence.
You will work with the team to develop new and innovative concepts, models, methods, and practices, to deliver primary care services to meet the needs of the practice.

Collingwood Health Group is the largest practice in North Tyneside with a patient list of c.25000 over 3 sites. Each site is run separately with stable teams at each but are very much ONE TEAM.
Our multidisciplinary team includes 10 partners, 12 salaried GPs, 10 practice nurses, 4 pharmacists, 1 physio, social prescribers, 2 mental health workers, 1 Physician's Associate and a home visiting team made up from Primary care and Community care practitioners.
The skills of our administrative team are diverse and you would have your own Administrative Assistant.

Responsibilities of the Advanced Nurse Practitioner will include the following:
- Work independently but as part of a multidisciplinary clinical team.
- Provide care of patients with a range of both chronic and acute medical needs, providing subject matter expert advice.
- Be an Independent Prescriber and support patients in the use of their prescribed or over the counter medicines.
- Assess patients with undiagnosed, undifferentiated medical conditions and make treatment decisions, including ordering necessary investigations.
- Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence.
- Provide both face to face and telephone consults.
- Maintain accurate clinical entries within the electronic patient records, ensuring coding is effective and in line with QOF and practice policy using templates eg Ardens as appropriate.
- Request, collect, process and interpret pathology and other test results as required.
- Review, process and action documentation relating to patients where necessary.
- Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patient’s health.
- Support the team in dealing with clinical emergencies.
- Recognise, assess and refer patients presenting with mental health needs.
- Take part in vaccination programmes for adults and children as required.
- Contribute to practice targets (QOF etc.), complying with local and regional guidance.
- Liaise with external services/agencies to ensure the patient is supported appropriately.
- Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual).
- Contribute to implementation of health promotion and wellbeing programmes.
- Support the clinical team with all safeguarding matters in accordance with local and national policies.
