Job summary

An exciting opportunity to work with One Thornton Heath PCN, a well-established primary care network of six GP practices in North Croydon.

The role requires participation in all aspects of patient care through knowledge gained from training, working for One Thornton Heath PCN across six GP practices as part of their multidisciplinary healthcare professional teams and will be supported and supervised by a GP Supervisor. This is a key position within our developing PCN and involvement in routine care possibly including care homes ensures the post holder has the necessary breadth of training and ongoing clinical challenge.

Main duties of the job

The Clinical Pharmacist is very much a patient facing role and there will be plenty of opportunity to use your clinical skills to improve the health of the local community; this will include managing long term conditions, supporting hospital discharge prescribing arrangements, providing specific advice for those on multiple medications, clinical audits to improve patient care and supporting the GP practices involved to develop the clinical pharmacist role. Clinical supervision and mentorship will be provided.

About us

One Thornton Heath Primary Care Network is an established network of 6 GP practices with many years' experience of collaborative working.

All the practices are committed to providing high quality care for the 54,000 registered patients within the PCN. This new post will join the team of twleve 1TH PCN staff currently working successfully across the practices.

The post holder will be employed by a lead training practice on behalf of the PCN and the lead practice will provide regular support and clinical supervision, reporting to the PCN Clinical Directors and Business Lead

Qualifications

Essential

  • Completion of undergraduate degree in Pharmacy and registration with the General Pharmaceutical Council
  • May hold or be working towards the independent prescribing qualification

Experience

Essential

  • Minimum of 2 years' experience as a pharmacist, demonstrated within a practice portfolio
  • Have an experience and an awareness of common acute and long-term conditions that are likely to be seen in general practice
