As a Frailty Practitioner you will work very closely with the multi-professional teams who together provide excellent standards of assessment and care to our frail patients within NSG Network which is made up of 5 practices including Alney Practice, Churchdown, Longlevens, Brockworth and Hucclecote Surgeries.
As part of a small team of Frailty nurses you willtake a prominent role in developing and maintaining a high quality service to a rising elderly population within the NSG area.
The successful candidate will be vital for the ongoing development of this role within a specialty that is undergoing rapid and exciting development. We welcome applications from capable and enthusiastic individuals who have the necessary skills and passion to work with us and our multi-disciplinary team including Social prescribers to provide this valuable service.
Main duties of the job
To provide expert clinical case management for frail patients who may have multiple long term conditions and are at risk of deteriorating health that may result in declining clinical quality of life or avoidable hospital admission or unnecessary length of hospital stay.
- Supporting and working with carers and wider family members.
- Develop relationships with staff within the practices, ICT, Rapid Response and social care relating to them on a day-to-day basis.
- Undertake comprehensive assessment of the physical and psycho-social care needs of frail patients who may also have complex chronic disease. This will involve using a Single Assessment Process, gathering and interpreting information, performing tests and analysing the results.
