Job summary

We are recruiting two 0.8WTE to 1.0WTE First Contact Physiotherapists to work as part of our multidisciplinary team across our five practice Primary Care Network. This will be a new role within our PCN and an exciting opportunity to help shape and develop a FCP service. The post holders will see patients from all five practices however they will have a host practice and become embedded in that team as well as the wider PCN team of over twenty-five employees. The post holders will have a designated clinical supervisor with protected supervision and debrief time.

Key Roles and Responsibilities:

  • To be the first point of clinical contact for patients presenting with MSK conditions or symptoms, providing the patient with the primary assessment, diagnosis and management options for their condition.
  • Use advanced clinical practice skills and clinical reasoning to provide comprehensive diagnostic and treatment support to patients who present with complex musculoskeletal conditions and/or multiple pathologies and/or mental health and/or pain management needs.
  • To be the link between primary, community and acute services, when the patient is managed via primary care ensuring an integrated care pathway for individual patients that meets their needs.lead on and develop effective communication between primary care services/GP practices and other relevant care providers.
  • Provide leadership and support on MSK clinical and service development across the PCN.
  • Provide highly specialist advice on issues ranging from the provision of expert opinion on individual patient treatment options to be a primary contributor to MSK services and related pathway development.

As a PCN we have well established Pharmacy and Living Well (Social Prescribing, Frailty and Care Coordinators) Teams. We are currently expanding our paramedic service and have recently recruited and Mental Health Practitioner. This year we have invested in the management and leadership of Network to support and grow the services we provide across all our practices.

We are a very friendly team and look forward to welcoming FCP's and working with the post holders to develop this new and exciting service.
