Job summary
We are currently looking for a part time nurse practitioner to join our friendly team.
The candidate will provide direct input in the care of practice patients to include, treatment, preventative care, screening and patient education.
The candidate will be competent and confident in managing people with long-term health conditions as well as patients with acute and chronic conditions,
The candidate must be skilled at time-management and be able to recognise and work within their own competence and professional code of conduct as regulated by the NMC.
The practice holds regular meetings including a monthly clinical support meeting where clinicians will discuss any NICE guidelines, clinical protocols, individual clinical cases, etc. Other regularly monthly meetings include Significant Events, Prescribing, Safeguarding, and MDT meetings to discuss the frail and elderly. The practice employs an outreach nurse to work with these patients in their own homes.
Main duties of the job
The candidate will see patients in clinics as well as carry out home visits, when necessary, and they will be expected to visit nursing homes as and when required. They will need to be able to diagnose and manage both acute and chronic conditions, integrating both drug and non-drug based treatment methods into a management plan. The candidate will see patients ranging from the very young to the elderly and provide assessment, screening, referrals to secondary care, when necessary, and health education advice. The candidate must maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development and disseminate learning and information gained to other team members in order to share good practice. They must be able to assess own learning needs and undertake learning as appropriate.
About us
The Lawns Medical Practice is one of 2 GP practices located in Diss. We
have 3 GP Partners, 1 Managing Partner, and a fantastic nursing team comprising
of 1 Advanced Nurse Practitioner, 3 Practice Nurses, 1 Outreach Nurse and 2
HCAs. We have a great team of admin and dispensary staff and we dispense to
approximately one third of our 7100 practice patients. We share our building
with Parish Fields practice and also with a large selection of community teams,
including nursing and physio. We all have a good relationship and try to help
and support each other. Working alongside the community teams and our resident
Social Prescriber is extremely valuable in being able to ensure that our
patients have the full package of care that they may need.
COVID-19 has changed the way we work but we are a strong team and we are
coping well with all of the many changes we have had to implement. Our aim is
to always ensure that our patients and staff feel comfortable and safe and we
have achieved this.
