Job summary
Saltash Health Centre is looking for a Mental Health Practitioner to
join our friendly practice team.
You will work alongside GPs, Nurses, Paramedics, Clinical Pharmacists, Social Prescribers and Link Workers, as well as the wider Mental Health teams.
The post holder of Mental Health Practitioner, acts
within their professional boundaries, working alongside the general practice
team at Saltash Health Centre. They will
work for the benefit of patients, providing and maintaining a high standard of
care for patients with mental health needs.
They will provide assistance and advice to the GPs and other members of the
primary healthcare team in managing the mental wellbeing of the practice
population.
The
post holder will be an integral part of the General Practice team, as well as
part of a wider multidisciplinary team.This includes building relationships
with other community providers and mental health services.

The role:
Work as part of a multi-disciplinary team in a
patient facing role to clinically assess and treat
patients using expert knowledge of mental health.
Assess all patient contacts with
primary care presenting with mental health concerns through clinical triage,
face to face appointment at the practice or via video or telephone appointment.
Provide
the interface between primary care and mental health services, helping patients
with the transition between services

all clinicians within the practice are encouraged to contribute to. We are a
well-developed Primary Care team including Paramedics, Pharmacists, Practice
Nurses, Mental Health Practitioner, Social Prescribers and First Contact
Physiotherapists. Our practice has
developed clinical triaging through AccuRx that is being successfully used by
our patient population. We are an
established training practice and value education and training. We welcome
foundation doctors, specialty trainees, and support training for other allied
health professionals.

in Cornwall which provides everything you need for a fantastic place to live and
work with beautiful surrounding countryside and coast on your doorstep.

Health and wellbeing of the practice team is paramount. Our Practice
ethos is to deliver excellent patient care and a great place to work. We are an
enthusiastic, sociable and stable team of 3 GP Partners, 6 Salaried GPs, 4
Practice Nurses, 3 Paramedics and a Mental Health Practitioner, a large team of
admin staff, plus additional ARRS resource including physiotherapists, clinical
pharmacists, and social prescribers.

Qualifications
Essential
- Current professional registration: Registered Mental Health Nurse,
- Completion of an undergraduate degree in nursing or equivalent Registered Nurse (Mental Health) and registration with the Nursing and Midwifery Council.
Experience
Essential
- Community experience in mental health services.
- Experience of providing a range of evidence-based interventions to people with a variety of mental health problems and serious mental illness.
- Minimum of a 2 years experience as a nurse, demonstrated within a practice portfolio
Desirable
- Experience of working in Primary care/GP practice
