Job Purpose:
The post
holder is responsible for the delivery of practice nursing care to the practice
population. Supported by the nurse
manager within the practice, they will deliver care within the boundaries of
their role, offering a holistic approach and focusing upon supporting patients
to be healthy, monitoring of long-term conditions, health promotion and screening
activities. The post holder will work
collaboratively with the general practice team to meet the needs of the
patients, supporting the delivery of policy and procedures and providing nurse
leadership as required.
Duties and
Responsibilities:
Professional Responsibilities
Participate directly in
the delivery of primary care treatment and health and wellbeing services to the
practice patient population.
Formulate reports as required for the practice.
Support the implementation of evidence-based guidelines
in conjunction with the practice nursing team.
Promote the continuing professional development of
yourself and other nurses & HCAs within the practice.
Participate in an annual appraisal.
Clinical
Undertake chronic disease management including
respiratory, diabetes and chronic heart disease as appropriate to the post
holders level of training and experience, and in accordance with national and
local guidelines.
Consulting for minor health problems through eConsult online
consultation platform and obtaining prescriptions where necessary. The assessment team will always be available
for advice and will see patients if necessary
Administer appropriate vaccines to adults and children according
to up to date Department of Health guidelines including vaccinations for
foreign travel
Family Planning advice and help including checking of IUCDs
Monitoring and care of post menopausal women including those
taking HRT
Cervical cytology
Giving health promotion advice to patients during consultations
as appropriate
Monitoring of long term conditions
Wound care / ulcer care
Removal of Sutures
Urinalysis
ECG recording
Blood Pressure measurements
Asthma/Peak Flow Readings/Spirometry
Venepuncture
Ear syringing
Chaperoning and assisting patients where appropriate who are
being examined by another clinician
Requesting pathology tests, for example urine culture, swabs
Maintaining stocks of medicines, drugs, vaccines, dressings and
equipment and re-ordering as necessary
Liaison with other agencies, including Acute and Community
Trusts
Ensure consultations and information are recorded in the
computer records as appropriate
Attend and participate in any Practice meetings when required
Able to recognize and manage anaphylaxis according to current UK
guidelines
Able to perform cardio-pulmonary resuscitation according to
current UK guidelines
Any other delegated duties appropriate to the post
Doppler and compression bandaging
Scope:
Your key responsibility is to provide and maintain a high
standard of nursing care for patients as well as providing nursing assistance
to the doctors and other members of the health care team in a range of varied
work activities described above. It is
essential that you have a pleasant manner, are calm and have good interpersonal
skills. A flexible approach is encouraged and you will be expected to work as
part of the team.

Registered Health
Professional
All employees of Saltash Health Centre who are required to register with
a professional body, to enable them to practice within their profession, are
required to comply with their code of conduct and requirements of their
professional registration.
Risk Management
It is a standard element of the role and responsibility of all staff of
Saltash Health Centre that they fulfil a proactive role towards the management
of risk in all of their actions. This
entails the risk assessment of all situations, the taking of appropriate
actions and reporting of all incidents, near misses and hazards.
Records Management
As an employee of Saltash Health Centre, you are legally responsible for
all records that you gather, create or use as part of your work within the organisation
(including patient health, financial, personal and administrative), whether
paper based or on computer. All such
records are considered public records, and you have a legal duty of confidence
to service users.
