Job summary
We are seeking a motivated,
innovative and experienced Care Co-ordinator with proven leadership skills, in
an autonomous position, to join our Enhanced Health in Care Homes
Multi-Disciplinary Team (MDT).
Main duties of the job
The post holder will be employed
by Cleveland Practice on behalf of the practices in the Trent Care Network.
As the Care Co-ordinator you
will work very closely with the PCN Advanced Clinical Practitioner Nurse, the Practices
and the Multi-Disciplinary Team within the PCN. The role is pivotal in ensuring
all care home patients receive the best possible care and service. The Care
Coordinators role will support the Lead GP the Advanced Clinical Practitioner
Nurse and the Multi-Disciplinary Team in coordinating all key activity
including access to services, advice, and information, and ensuring health and
care planning is timely, efficient, and patient-centred. The role will include
supporting digital initiatives and includes responsibilities for the co-ordination
of the patients journey through primary care.
The role will be based in
multiple locations across the PCN area.

consists of 5 GP surgeries in Saxilby, Gainsborough and Hibaldstow. We cover a population size of approximately
40,000 patients with a range of diversity.

- Passed training requirements as outlined by the Personalised Care Institute which will be live from April 2020 (or be prepared to enrol and complete the next available course)
- Evidence of continuing professional development

- NVQ Level 3 or equivalent qualification / experience within a business administration field
Experience

- Experience in care coordination or clinical administration
