We have an opportunity for an Advanced Nurse Practitioner to work
as part of our excellent and friendly clinical team in supporting the duty/same
day service and management of long term conditions.
We want a caring healthcare professional who takes a pride in
delivering a service that is of a high standard.
The post is initially for two days a week either a Tuesday,
Thursday or Friday. The hourly rate will be dependent on experience.
As a member of the team, you have access to the NHS pension
scheme for which the employer contributions are 23.7% and the Blue Light card,
offering a wide range of exclusive discounts to public sector workers. A
full-time post would have 35 days holiday, including bank holidays (pro-rata
for part-time employees).

Main duties of the job
We are looking for an advanced nurse practitioner to work
autonomously in practice within the practice team to support the duty/same day
service as well as providing care for patients with long term conditions.
You will be an integral part of both our practice and the wider
multidisciplinary teams operating across primary care.
Candidates who are qualified as
advanced care practitioners but are not yet qualified as an independent
prescriber must be prepared to work towards a non-medical prescribing
qualification.

Practitioner has the following key responsibilities in delivering health
services to our patients:
- Practice both independently within
scope of practice and in partnership with the multidisciplinary team (MDT) under
the clinical supervision of a General Practitioner, ensuring interventions and
treatments are undertaken in line with best practice.
- Work as part of the practice
and wider multidisciplinary teams to support the delivery of the duty/same day
service and reviewing complex patients with long term conditions and sharing
expertise to improve patient care.
- Be an independent prescriber, using
prescribing and deprescribing skills to optimise the use of medicines to
improve patient outcomes.
- Undertake home visits for the
practice.
- Perform at an advanced level

of the patient, analysing complex clinical signs, patient history, and
investigation to establish a clear care plan, including the generation of
