Practice Nurse

Job summary

The Practice Nurse is part of the Nursing Team within Kingswood Medical Group (KMG) and will be working with and supported by clinical and non-clinical colleagues, as part of a multi-disciplinary team (MDT).

The postholder will need drive and enthusiasm, as well as an ability to problem solve.

The role will require flexible working across Kingswood and Carfax to support the nursing team provide a holistic nursing provision to our patients.

Main duties of the job

In the provision of patient care the Practice Nurse will:

Act as an autonomous clinician and take responsibility for the assessment, treatment plan and/or to appropriately refer patients to specialist services if required.

Be compliant with the NMC Code of Professional Conduct and any associated national guidelines and abide by their guidance.

Take a comprehensive patient history, carry out physical examinations and use expert knowledge and clinical judgement to identify the potential diagnosis.

Refer patients for investigations and/or the appropriate specialist where appropriate.

Provide chronic disease management in line with the quality and outcomes framework (QOF) and in accordance with NICE guidelines.

Carry out assessment and clinical decision making using skilled evidence based knowledge.
