Job Summary
We
are looking for applicants with previous admin experience to join our small
Care Home team to provide excellent admin support to our Clinicians and team
members.
You
will need to be friendly and approachable with effective communication skills,
able to build good working relationships within a fast-paced environment. As
well as being a team player, you will need to demonstrate sensitivity and
confidentiality due to the nature of our services, work autonomously and
promote a flexible approach and strong work ethic to prioritise, multi-task,
and delegate.
We
are looking for an organised, reliable administrator to join the Care Home
team. They will support the wider team with general admin tasks and have a high
level of organisation and the ability to adapt to new tasks as and when they
arise.
Main duties of the job

Managing the Care Home team

Answering and actioning
requests that come through the teams phone line
Data entry and updates onto
Excel Spreadsheets and patient records
Generating reports for the
Clinical Team
Formulating templates to
assist the team
Creating how-to guides for
complex templates used by the team
Supporting the team with IT
requests
Spreadsheet management
Liaising with care homes and
other services
Send referrals to other
services on behalf of residents of care homes
Assisting members of the team
with home rounds; notetaking, typing up, updating systems
Take meeting minutes from
weekly MDT and ad hoc meetings
Provide admin support to help
the clinical team manage residents who are admitted discharged from hospitals
Manage blood requests for
residents of Care Homes
Support with coordination of
Chronic Disease Management reviews
Input data on Personalised
care and support plan templates
Support team in generating
EPaCCS on EMIS
Other admin tasks scanning,
printing, uploading documentation
Monthly tasks pull
population reports, gather data
And any other duties which are
appropriate to the role and support the wider team
About us
Primary Care Networks
A Primary Care Network (PCN) enables the provision of proactive, accessible, coordinated and integrated primary and community care, improving outcomes for patients. They are formed around natural communities based on GP registered lists, serving populations of around 30,000 to 50,000.
The Lytham St Annes Primary Care Network includes, 5 GP Practices, Holland House, Fernbank, Parcliffe, Poplar and Ansdell, serving a population of 53,772 registered patients.
