Lytham St Annes
Primary Care Network (PCN) is recruiting a Health Care Assistantto support patient care in line with the
Network Contract Directed Enhanced Service (DES) with a focusing on areas of
the Enhanced Health in Care Home Framework.
We are seeking a Health Care Assistantpreferably with
previous experience of working in the community. The post holder will work as
part of a multi-disciplinary team in a patient-facing role. The post holder will
support residents in care homes completing Chronic Disease Management (CDM)
reviews and other HCA tasks.
The post holder will help reduce GP service demand by conducting Care home visits and Chronic Disease Management reviews for Care home residents.The post holder will work to integrate practices and community services to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving access to treatment. The post holder will need to work flexibly to meet the needs of the care home team and the GPs within the primary care network.
Main duties of the job
The following are the core responsibility of the Health Care Assistant. There may be on occasion, a requirement to carry out other tasks; this will be dependant upon factors such as workload and staffing levels:
- Complete CDM Reviews and documentation
- Carry out baseline observations such as pulse oximetry, blood pressure, temperature, pulse rate, respiration rate and recording findings accurately.
- Facilitate routine and 24-hour BP monitoring, advising patients accordingly.
- Carry out weight and BMI checks as directed.
- Undertake venepuncture.
- When trained, carry out ECGs as requested.
- When trained, carry out end user equipment forms.
- Deliver opportunistic health promotion where appropriate.
- Participate in local initiatives to enhance service delivery and patient care.
- Support and participate in shared learning with the PCN.
- Support health promotion programmes.
- Ensure specimens are recorded and ready for onward transportation.
The post holder will need to hold a full driving license and be prepared to travel between locations to support the remit of the role.

The PCN comprises of 5 Practices operating from 4 premises over the geographical footprint of Lytham St Annes and the villages of Warton and Freckleton. Working relationships within the PCN are well established as the Practices have already been working collaboratively.
The PCN serves a
diverse population with relatively low deprivation with complexity from an
above average elderly population and above average number of residents in care
homes. The PCN is leading the way in network working with a Care Home Team
already in place.
To work together across G.P. surgeries and Network teams within the Lytham St Annes area, for the benefit of patients and the wider community.
Lytham St Annes PCN will collaborate to provide excellent care and support for patients and practices, helping them access and navigate to the most appropriate healthcare to meet their individual needs.
