The 6 GP practices of Lytham St Anne's that make up Lytham St Annes Primary Care Network (LSA PCN) are recruiting a Band 7 AfC Pharmacist to add to our growing PCN team.
The ideal candidate will have primary care experience and must be a prescriber.
This is an exciting role in a new team and will be an incredible opportunity for the right person to be involved in shaping primary care locally.

Main duties of the job
We are seeking a
Clinical Pharmacist preferably with previous experience of working in primary
care. This exciting new post comes with
the benefit of being situated in a training practice with many years
experience of training Gps, nurses and other health professionals thus
providing a support rich environment to establish this new way of working. Rarely
in the NHS does an opportunity arise to be part of a brand new team and way of
working at the cutting edge of primary care medicine. The post holder will work
as part of a multi-disciplinary team in a patient-facing role. The post holder
will take responsibility for areas of chronic disease management within
practices and undertake clinical medication reviews to proactively manage
patients with complex polypharmacy.
About us
The PCN comprises of 6 Practices operating from 5 premises over the geographical footprint of Lytham St Annes and also the villages of Warton and Freckleton. Housed in modern premises with other community services and community pharmacies co-located. Working relationships within the PCN are well established as the Practices have already been working collaboratively for 3 years as Lytham St Annes Neighbourhood.
You will be joining a team of PCN pharmacists and technicians already in post and working from the PCN offices based at Lytham Primary Care Centre.

The post holder
will provide primary support to general practice staff with regards to
prescription and medication queries. They will help support the repeat
prescriptions system, deal with acute prescription requests, and medicines reconciliation
on transfer of care and systems for safer prescribing, providing expertise in
clinical medicines advice while addressing both public and social care needs of
patient in the GP practices.

will provide clinical support on medicines optimisation and quality improvement
and manage some aspects of the quality and outcomes framework and enhanced
services. The post holder will also take responsibility for areas of chronic
