Main duties of the job

Essential Qualifications

Diploma in or Respiratory Management

Experience of working in Primary Care for 2 years.

Administering Vaccinations under PGDs

Chronic Disease Management in accordance with NICE/ evidence based practice.

Ability to titrate medications following protocols.

Health Promotion.

Knowledge of Chronic Disease.

Understanding of QOF and CQC requirements.

Ability to support in the absence of the lead nurse.

Evidence of continued professional development.

Good time management.

Work well in a team

Proactive.

Competent IT skills.

Good communication skills.

Self-motivated.

Job description

Job responsibilities

The post holder will undertake a range of clinical skills developed through experience. They will deliver high quality nursing care in General Practice as a self-directed practitioner, working with the GPs and other nurses to meet the health needs of the patients.

MAIN DUTIES AND RESPONSIBILITIES

The provision of nursing treatment to patients in participation with the GPs, or independently, according to agreed and documented protocols.

The provision of a comprehensive assessment of a patients nursing requirements via the development, implementation and evaluation of an individual programme of care according to need.

Provide a contraception and sexual health service

Carry out screening procedures, eg. cervical cytology.

Advise on general health care and minor ailments, with referral to a GP as necessary.

Have knowledge and overall nursing management of chronic disease, particularly COPD/Asthma and be responsible for the setting up of systems for regular review.

Assist in the formation of protocols and procedures/guidance.

Advise on the promotion of health and the prevention of illness, through the provision of health education and lifestyle counselling.

Review patients laboratory results and initiate treatments according to practice protocol.

Able to document consultations according to NMC guidelines and have ability to obtain and document consent either verbal or written.

Awareness of statutory and local safeguarding procedures, including systems of referral and have the ability to recognise signs and symptoms of abuse.

Have ability to recognise and manage anaphylaxis according to current UK guidelines.

Have ability to perform cardio-pulmonary resuscitation according to current UK guidelines.

Identify and implement change in line with evidence based practice and national/local guidelines.

Ensure confidentiality of information gained at work is preserved at all times.

In the absence of other members of the nursing team, have the ability to provide a high standard of nursing care for the patients within the practice by continually updating and improving knowledge in areas not routinely undertaken.

Provide management and screening in line with current evidence-based guidelines in cervical cytology, reproductive and sexual health /contraception.

Offer a holistic approach to travel health and provide comprehensive advice to patients prior to travel including vaccinations and medicines, safe sex, food hygiene, sun protection.

Ensure vaccines are administered under patient group directives.

Ensure infection control guidelines are maintained.

Monitor and manage maintenance of stock and equipment to include refrigeration, and emergency equipment.
