Job summary

The role of the Clinical Pharmacist will work across up to two of the Practices within our Primary Care Network. Our Practices are Alconbury, Buckden,Kimbolton, Almond Road and Wellside.

You will be part of a friendly and proactive Clinical Pharmacy team, comprising Clinical Pharmacists and Pharmacy Technicians. These roles have been firmly embedded within Practices for a number of years, and we are always seeking to further develop the role of our Clinical Pharmacists.

You will need to be an Independent Prescriber,having also completed the PCPEP course.

Main duties of the job

We are currently seeking a dedicated Clinical Pharmacist (Independent Prescriber) to join our Primary Care Network team . This position offers an exciting opportunity for an individual with a passion for patient care and medication management.

Duties:

- Action requests for changes in medication and medication reconciliation

- Review patient medical histories and medication regimens to ensure safe and effective drug therapy

- Undertake medication reviews

- Provide drug information and counselling to patients on proper medication use

- Conduct medication reconciliation and ensure accurate documentation of pharmaceutical care interventions

Qualifications:

- Bachelor of Pharmacy degree or equivalent qualification

- Completed the PCPEP course

- Registration as a pharmacist with the General Pharmaceutical Council (GPhC)

- Proven experience in a Primary Care setting, with knowledge of EMIS Web and SystmOne

- Strong understanding of anatomy and its application in patient care

- Excellent communication and interpersonal skills

- Ability to work effectively in a multidisciplinary team

- Proficiency in medication administration procedures

Our Primary Care Network has been in existence since 2019. In addition to a Clinical Director and Primary Care Network Manager, we employ more than 20 staff.

Our services are delivered across our five Practices. In addition to Clinical Pharmacists and Pharmacy Technicians, these include First Contact Physiotherapists, Social Prescribers, GP Assistants, GPs, Paramedics.

We consider ourselves to be an innovative and forward thinking organisation, working collaboratively as a multidisciplinary team to deliver excellent care to our patients.
