Job summary
We are looking to recruit an experienced, dynamic primary care pharmacist who can prescribe. The role will be based in our practice alongside our medicine management team which includes 3 pharmacists, and 2 pharmacy technicians and the support of medicines management GP Partner.
We are looking for a practice pharmacist to work closely with the team to ensure we deliver an effective, efficient and quality medicines management service to our patients and our team.
Main duties of the job
The Pharmacist will undertake the following tasks.
Medication reviews Face to face, remote and e-consultations) and proactively manage patients ( some with complex polypharmacy). This will include patient education,prescribing and monitoring as per guidelines and within professional competencies.
Work closely with the wider team, advising and supporting on medicine management issues.
Undertake prescribing audits.
Ensure that the practice is encouraged to promote cost effective prescribing.
Ensure medicines are prescribed and monitored as per the most current guidelines.
Ensure that the practice systems for drug safety and drug safety monitoring are adhered to at all times. This work will include regular safety audits.
Share knowledge and learning with the wider teams.
Attend regular practice meetings.
Represent the practice at outside events.
Contribute to achieving high IIF/ QOF targets.
Undertake medication reviews for housebound/care home patients as needed.
Take an active role to help/ support practice initiatives.

We are large forward thinking training practice, which prides itself in providing quality care for our patients and the education and professional development of all of our team.
We have around 30,000 patients over 4 sites. Our patient demographics are wide and diverse, as we have a sites at Chaddesden, Borrowash , Oakwood and an on campus at the University of Derby.

We have weekly practice meetings which the successful candidate will be invited to attend and share their knowledge with the wider team.
Our practice works closely with 2 nursing homes and 2 residential homes.
We are part of Oakdale Park PCN, which is made up of Park Medical Practice and Overdale Medical Practice.

Reconcile medicines following hospital discharge and work with patients and the community pharmacists to ensure that the patients receive their medication post discharge.
Reconcile medicines from clinic letters with the support from the pharmacy technicians.
Support long term condition reviews
Care home ward rounds to carry out medication reviews and de-prescribe as appropriate.
Contribute to reductions in medicine related hospital admissions and re-admissions by supporting patients to get the best outcomes from their medicines.
Identify and address medicine related issues.
Act as the interface with community and hospital colleagues.
Manage patients and healthcare professional medicine queries.
In collaboration with the management team Investigate any complaints involving medicine issues.
Increase patient quality and patient safety of prescribing through mechanisms such as audit and PDSA cycles. The share the findings with the wider team.
Contribuite to service improvement within enhanced services QOF and IIF
Implement drug withdrawals and alerts eg MHRA
Work with the primary care professional and patients to implement NICE and other evidence based guidelines.
Contribute to multi-morbidity reviews and multidisciplinary reviews.
Provide medicine related information and training to practice staff.
Act as a source of medicine information for all of the practice team and patientss eg around supplies, doses, side effects

- To be responsible for own professional development and maintain competence withing own clinical field. Post holder will also need to keep up with CPD requirements as specified by GPhC

- Holds a prescribing qualification
- has completed the CCPE pathway
