Job summary

Due to the retirement of our long serving Nurse Team Manager, we have an opportunity for a forward thinking experienced Primary Care Nurse (minimum 3 years) to join our practice team.

Interviews will be held on Tuesday 14th June 2022

Main duties of the job

The successful candidate will manage, support and direct our Nursing team, as well as being an integral part of the practices management team working together to take our practice forward. The ideal candidate will have a special interest

About us

We are a busy GP practice based in Chaddesden, Borrowash, Oakwood and the University of Derby. We currently have 29,000 patients Our aim is to provide quality patient care of our wide range of patients. We are part of Oakdale Park PCN, a PCN made up of two practices and we are a very active member of Primary Care Plus. The post holder will be required to travel between all sites.

HOURS: FULL TIME

REPORTS TO: PRACTICE MANAGER

ACCOUNTABLE TO: GP PARTNERS (Clinical)

PRACTICE MANAGER (Organisational / Administrative)

Summary of the position As a clinical member of the management team, to be responsible for the safe and effective delivery and future development of the overall general practice service, working towards achievement of practice aims and objectives.

Working within the NMC code of conduct the post holder will take a senior role within the nursing team, using primary care experience to manage and develop the nursing service, provide nurse interventions and influence the continued development of the nursing service.

To provide a practice nursing service to patients, including chronic disease management and other specialist areas. To deliver evidence-based practice, demonstrate critical thinking and possess skills in clinical decision making in order to deliver high standards of patient care.

To lead the nursing team currently comprising of 8 practice nurses, 3 health care assistants and 1 phlebotomist.

To work collaboratively with the whole general practice team to meet the needs of the patient

including general practitioners, clinical practitioners, primary care co-ordinators, pharmacists, and pharmacy technicians.

To establish excellent working relationships with the wider CCG community for example, The Primary Care Network and specialist nurses, to ensure high standards of patient care whilst providing and supporting ongoing opportunities for learning and development within the Practice Nurse Team.

Qualifications

Essential

  • Registered general nurse with current NMC registration
  • Evidence of relevant up to date CPD
  • Diploma in a Long-Term
  • Condition management e.g. Asthma / COPD/ Diabetes/ CHD
  • Recent or current experience in Primary Care

Desirable

  • Management/ leadership qualification or experience

Experience

  • Relevant transferable skills such as wound care management, cervical cytology, family planning, adult/ child immunisations and vaccinationsAbility to demonstrate commitment to high quality patient care and service innovation
  • Experience of QOF
  • Strong decision maker Demonstrates excellent, organisational, time management and motivational skills

  • Experience in managing staff (staff absences, conduct, performance)
