Job summary

South Bristol locality has been chosen as one of the 43 places to be included in the first phase of the National Neighbourhood Health Implementation Programme (NNHIP), a new and exciting national project. Within South Bristol, there are three pilot projects running, aligned with the local Primary Care Network (PCN). Partners in the Swift PCN geography originally agreed to focus their project on building on the successful delivery of mental health group sessions provided by Mental Health Workers based in the GP practice, jointly funded by Avon and Wiltshire Mental Health Partnership (AWP). Due to the recent allocation of funding, the original proposal has developed into four bespoke sessions focusing on the contribution of positive mental health to wellbeing, and four further sessions hosting partners to help address the wider determinants of health such as housing and food poverty. The target cohort across South Bristol is aligned with the recommendations of Healthier Together 2040 report, with the cohort being adults aged 20-64 diagnosed with at least three of the following: diabetes, hypertension, pain, anxiety and depression.

For the project's success, a project manager is required to oversee and lead on the implementation of the project. The project manager will work closely with the care co-ordinator to lead and deliver on the project-associated work. Whilst the project manager will be employed by the PCN, they will be working closely with local partners.

Main duties of the job

Managing project finances with the PCN Network Manager and Finance Manager

Reporting project progress at agreed intervals to the PCN Network Manager and Swift NNHIP project board

Maintaining an up-to-date suite of project documentation

Managing data collection and contributing to the development of data collection systems

Preparing reports and information to be shared internally and externally

Liaising with colleagues to learn from previous projects, implementing best practice and connect this project to the wider piece of work

Developing and maintaining strong relationships with internal and external stakeholders

Participating in and support all relevant internal and external meetings

Liaising with multiple individuals in the community including those leading Voluntary, Community and Social Enterprise (VCSE) organisations, those with lived experience and those who are the target of our interventions

Managing and delivering initiatives within the project plan

Working closely with and allocating work packages to the project Care Co-ordinator

Working closely with the PCN Network Manager to ensure the projects timely delivery

Working closely the locality PCN managers and locality managers

Contributing to the implementation of the wider NNHIP programme

About us

Swift PCN covers a large area of South Bristol, including Hartcliffe and Withywood, Knowle West, Whitchurch, Bishopsworth, Bedminster and Hengrove. Swift PCN sits within the South Bristol Locality Partnership who are overseeing this project, alongside Sirona Care and Health, Avon and Wiltshire Mental Health Partnership, Voluntary Community and Social Enterprise organisations (VCSE), care providers, Bristol NHS Trust and BNSSG Integrated Care Board (ICB). The Swift PCN who will support the project manager, is made up of the PCN manager, PCN Administrator, Finance Manager and three Clinical Directors.

NHS England and the Department of Health and Social Care launched NNHIP in July 2025 and has recruited 43 places, including South Bristol. This is an exciting opportunity to be part of something new with national support and opportunity for innovation.

The project manager position is ideal for an individual who has great interpersonal skills, is confident in leading on change and wants to play a pivotal role in delivering neighbourhood health. The main responsibility of the position is managing delivery of project activity as determined by the project board. The project manager will be required to create work packages for the care co-ordinator and PCN administrator to ensure successful delivery. They will be supported by the PCN manager and partner organisations.

This position is for a fixed term of one year, with a pa salary of £40,716. The hours required are 22.5 hours per week (0.6FTE) and these can be worked flexibly, although attendance at key meetings will be required. Working will be hybrid, with some hours worked in the office and some worked at home.
