Job summary

We are seeking a compassionate, organised and motivated Health Care Assistant / Care Co-ordinator to join our busy GP practice. This is a varied and rewarding role that combines traditional Health Care Assistant duties with the additional responsibility of care co-ordination, supporting patients to navigate services and receive high-quality, joined-up care.

The successful candidate will work as part of the multidisciplinary primary care team, delivering essential clinical support such as phlebotomy and health checks, while also playing a key role in coordinating care for patients with long-term conditions, complex needs or vulnerabilities. This includes liaising with clinicians, community services and external agencies to ensure patients receive timely, appropriate and person-centred care.

This role is ideal for someone who is patient-focused, proactive and highly organised, with excellent communication skills and a genuine commitment to improving patient outcomes within primary care.

Main duties of the job

The successful candidate may already be practising as a Healthcare Assistant or may have worked in a similar care environment and have an interest in Primary Care and developing their skills further. A core element of the role is phlebotomy (blood taking) but the remit of the job is varied.

The Lennard Surgery team is comprised of 9 doctors, 5 nursing staff, pharmacists, physio, paramedic, and an administrative/management/reception team of 17.

Health Care Assistant Duties

  • Undertake clinical tasks under the direction of registered clinicians, in line with training and competencies

  • Carry out routine observations including blood pressure, pulse, height, weight, BMI and urinalysis

  • Perform phlebotomy and ECGs (if trained)

  • Support chronic disease management clinics (e.g. diabetes, asthma, COPD)

  • Carry out health checks, wound care, dressings and compression bandaging

  • Prepare patients and clinical areas for procedures, ensuring infection prevention and control standards are maintained

  • Accurately record clinical information in the patients electronic record

  • Manage stock, equipment and clinical supplies as required

Care Co-ordinator Duties

  • Act as a point of contact for patients, families and carers to help navigate services
