Main duties of the job

This is a clinically focused role involving:

  • Assessment, diagnosis and management of acute and chronic conditions
  • Face to Face clinical sessions
  • Telephone consultations
  • Home visits for nursing home and housebound patients
  • Working autonomously within NMC competencies and scope of practice.
  • Contributing to high quality patient centre care
  • Prescribing within national and local guidelines
  • Appropriate referral, escalation and safety netting
  • Proactive and preventative care following local and national targets, such as Quality and Outcomes Framework (QOF) and Bolton Quality Contract (BQC) and Beyond Core Contract (BeCCoR) reviews
  • Timely clinical documentation, using SNOMED codes

We operate a total triage system led by the GP Partners, who triage all appt requests to help ensure all staff see patients appropriate for their experience and expertise as well as their personal learning and development needs. We pride ourselves on being a positive learning environment. Daily 30 minute clinical meetings ensure shared learning, case discussion and strong communication across the team.
