Job responsibilities
The Advanced Clinical Practitioner will work as an autonomous clinician within a GP practice operating a total digital triage model. The post-holder will assess, diagnose, and manage patients with a range of acute and long-term conditions, ensuring patients are directed to the most appropriate care in a timely manner. The role supports safe, effective service delivery while contributing to service development and team working.
Key Duties and Responsibilities
- Independently assess, diagnose, and manage patients presenting with acute illness and long-term conditions
- Undertake clinical triage within the digital triage system, prioritising and directing patient care appropriately
- Provide clinical support to the on-call GP within the digital triage hub
- Prescribe medication in line with scope of practice, clinical guidelines, and local policies
- Make appropriate referrals to secondary care and community services
- Maintain accurate and timely clinical records using the practice clinical system
- Participate in clinical meetings, peer review, and multidisciplinary discussions
- Contribute to service development, quality improvement, and practice initiatives
- Support teaching, learning, and supervision of trainees and other staff as appropriate
- Work within professional, ethical, and legal frameworks, maintaining registration and revalidation requirements
Professional Responsibilities
- Practice in accordance with NMC / HCPC codes of conduct and professional standards
- Maintain up-to-date clinical knowledge and skills through CPD
- Work within practice policies, procedures, and governance frameworks
- Contribute to a positive team culture and support colleague wellbeing
