Job summary

We are recruiting to the role of PCN Network Care Co-Ordinator to support the Newport & Central Primary Care Network (PCN).

We are a friendly and expanding network consisting of 4 GP Practices with a patient population of circa 57,000 patients. Member practices include Donnington Medical Practice & Shawbirch Medical Centre in Telford and Linden Hall Surgery & Wellington Road Surgery in Newport.

This is an exciting new role where the post holder will be the focal point for and work across data systems for the PCN to support systems related to the delivery of the Network Direct Enhanced Service contract , IIF targets and services.

The Pay scale offered till be between £11.50 and £12.60 per hour dependant upon experience.

Previous knowledge of General Practice is preferred but not essential to this role.

Access to the NHS Pension Scheme is available.

Main duties of the job

Primary Care Networks (PCN) are critical to the changing face of primary care within an expanding NHS system and the focal point of all locally developed planning and service delivery.

The PCN Network Care Co-ordinator will be responsible for undertaking a wide range of operational and data systems administrative duties, and assist in the delivery of services to our patient population.

The successful candidate will, as directed by the PCN Clinical Director and or PCN Manager, play their part in ensuring that the data recorded across the PCN clinical systems is, as far as possible, uniformly consistent by use of common SNOMED codes and templates for the recording of patient information, and support service delivery within our teams.

About us

Any candidate will be joining the PCN at an exciting time of expansion and development, having recently registered as a Limited Company.

We currently employ a team of Clinical Pharmacists, Pharmacy Technicians, Social Prescribers, First Contact Physiotherapists, Primary Care Paramedics and a Mental Health Nurse Practitioner to support patients and service delivery within member practice teams.

The PCN has had many achievements over the last 2 years with a growing workforce across all 4 surgeries.

Job responsibilities

The post holder, in conjunction with the Network PCN Manager, is responsible for carrying out any necessary IT searches and audits as required for the provision of information and data to support the delivery of the DES, achievement of IIF targets and any Information Governance requirements.

The post holder will coordinate and set up clinics for ARRS staff, including the extended hours service & home visiting service for patients across the Network. They will ensure data requirements within the PCN DES are met, and data on appointment availability and utilisation is provided to the PCN Board.

This job description is a summary of what the Network Care Coordinators job will entail, and the role may expand and change over time as the needs of the PCN evolve, the requirement of the Network DES and the priorities of the network population evolve.

KEY REQUIREMENTS OF THE ROLE

The following are the core responsibilities of the PCN Network Care Co-ordinator. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels.

  • Extended Access Responsibilities
  • Set up clinics and appointments on EMIS Enterprise for hub-based extended access appointments and other PCN-wide clinics, with a range of multi-disciplinary clinicians at a sufficient level to meet the requirements of the DES.
  • To book GPs, Nurses, and other clinicians (including ARRS staff) to work extended hours sessions,
  • Support clinicians working at the extended access hub with IT training and support as required.
  • Collect and collate data from practices relating to the internal and hub extended access scheme(s), monitoring practice & PCN achievement, and advising the Clinical Director and/or PCN Manager on performance.

Home Visiting Administrative Support

  • Support the PCN Primary Care Paramedics in managing the booking and coordination of visits across the Network.

Data Analysis Responsibilities

  • Utilise population health intelligence to proactively identify and work with cohorts of patients to deliver personalised care.
  • Work within the PCN to identify and manage a caseload of patients and, where required and as appropriate, refer people back to other health professionals within the PCN;
  • Support PCN and practice lead in facilitating searches.
  • Support the delivery of the Network DES, PCN-delivered services, IIF schemes, QOF, incentive schemes, QIPP and other quality or cost-effectiveness initiatives by providing data both at the PCN level and member practice level.
  • Maintain the PCN Ardens Dashboard, running monthly searches (or as agreed) to ensure that data is up to date, providing reports on a quarterly basis for the PCN Board and Management Board.
  • To undertake Population Management reporting as identified and required by the Management Board to support PCN work targeting specific cohorts of patients experiencing Health Inequalities.
  • Undertake any tasks consistent with the level of the post and the scope of the role,ensuring that work is delivered in a timely and effective manner.
