We have a post available for an Experienced
Part-Time, Practice Nurse.

32 hours per week - 4 x 8 hour days in line with practice
need.
(start date negotiable)

We are looking for a skilled, enthusiastic,
motivated individual who will share our ethos of commitment to high quality of
care in a long established practice.
Experience in Primary Care and Long Term
condition Management is essential.
Main duties of the job
You must be able to demonstrate some
experience of working in a Primary Care or similar setting.
We understand that not all applicants will
have all the long term condition qualifications but you will need to be able to
demonstrate you can deliver some of these standard reviews as this is part of
our core service. Training can be discussed at interview.
A smear taker would an advantage however,
we will be able to arrange this training if required.
We are lucky to have skilled HCA's working
in practice so standard reviews and appointments such as bloods, BP checks,
ECG's 24hr BP, dressings and other basic routine appointments are usually
provided by our HCA's. This frees up appointment time to enable our skilled
practice Nurses to deliver the more complex services.

Job Summary
As a member of the Primary
Health Care Team develop and deliver a high quality of service to the Practice
Population. To provide within competence framework, clinical assessment,
diagnosis, treatment and evaluation of care for the patients. They will also
participate in chronic disease management clinics, immunisation, cervical
cytology, health promotion and the day to day duties carried out by the nursing
team. They will work closely with the practice team to meet the needs of the
patients in line with Practice Objectives and National Guidelines. Contributes
to the overall Practice targets and objectives.
Core Behaviours/Knowledge
& Skills.
Must have undergone and
completed fully recognised Nurse Training. Have previous experience working in
a primary care setting. Required to work within the Code of Professional
Conduct and other directives as defined by the NMC. Excellent communication
skills both written and verbal are essential. It is desirable that the
applicant has some basic computer skills with some experience of clinical
systems for example System One, The applicant must be able to work as part of a
team, have the ability to organise and prioritise own clinical caseload on a
day to day basis. Responding effectively to patient need and ensuring patient
choice and ease of access to services. To comply with all practice protocols
paying particular attention to COSHH data protection, patient confidentiality,
health & safety, Infection Control, equality &diversity and customer
care at all times. The applicant must know when to seek direction from the
General Practitioners, Practice Manager.
Training and Personal
Development
The post holder will be
required to take responsibility in agreement with the Practice Manager and General Practitioners for his/her own personal development by
ensuring that Continuous Professional Development remains a priority. To
maintain their professional registration. Attend relevant courses and study
relevant to the post. To work flexibly as the different primary care nursing
specialities develop. Ensure they attend relevant meetings both internally and
externally. Training needs will be agreed and supported by an annual appraisal.
The post holder will undertake all mandatory training required for the role.
Time will be allocated within
paid practice hours for mandatory training and any other training that is
requested by the GPs to fulfil the role.
Job Outline
- To provide the choice of direct access to a
Practice Nurse in the practice for the general practice population, including home visisting as required
- To maintain effective working relationships
with colleagues within and external to the practice
- Liaise with all necessary attached staff
and external organisations. Such as Midwives, Community nursing teams,
Diabetic and Respiratory Nursing Specialists, Learning Disability Team
- Attend Multi-Disciplinary (MDT) meetings
regularly
- To provide an holistic, autonomous clinical
nursing service
- To demonstrate confidence to patients,
their relatives and carers by establishing a climate of mutual trust and
respect. Being aware at all times of the patients emotional, cultural and
social needs.
- Make professionally autonomous decisions
for which he/she is accountable
- Regularly assess and act on the views of
the patients, relatives and staff about the standards of care given.
- Undertake necessary invasive and non-
invasive diagnostic tests or investigations and interpret findings/reports
within the competency framework

- Urinalysis. Report any abnormalities to the
clinical team. Prepare samples for laboratory.
- Wound care and dressings. Basic and
recognising referral for specialist input
- Smoking Cessation. Basic support and
referral for specialist support
- Alcohol. Basic support and referral for
specialist support.
- Health Checks/new patient checks including
height/weight/BMI following in house procedures
- Infection control. To be aware of all
policies and procedures in relation to the prevention and control of
