Job Summary
Rutland Health PCN is a group of four high achieving practices based in the rural county of Rutland. Working with over 42,000 patients, we work collaboratively with Rutland County Council and community services to focus on local health needs and population health. Uppingham is a rural town, boasting a wonderful gym, glorious countryside, and great coffee shops. It is 1520 minutes from Leicester, with little traffic and wonderful people.
The successful candidate would be expected to work collaboratively within the network to understand current service models, identify challenges, and agree priorities for improvement. It will be essential for the post holder to liaise with other PCN Digital and Transformation leads and key individuals within the GP Federation and the ICB including IT / Digital Leads.
Using data quality improvement (QI) processes and optimising digital tools and innovation, the post holder will lead on planning and implementation of operational process to support Rutland Health PCN priorities which will improve the delivery of primary care services locally.
The ideal candidate will need to be proactive, have a passion for Primary Care and an understanding of the NHS digital landscape to assist Rutland Health PCN with its digital transformation agenda.
This is an exciting opportunity to lead transformation work across Rutland Health PCN and use digital technology where appropriate to enable change and maximise use of available tools.
The core objectives of the Digital and Transformation Lead role are to support Rutland Health PCN and its practices to deliver ongoing improvement to services along with providing a central source of leadership and management to aid service transformation for the PCN.
The remit of the role is broad and will require attention to detail in delivering high-quality day-to-day operations along with providing strategic leadership and management to the service delivery function, ensuring that a high quality and effective service is delivered.
Key Duties & Responsibilities
Responsibilities may include but are not limited to:
Strategic Planning
- To lead and develop a strategic plan that includes a robust digital infrastructure taking into consideration the populations health needs and health inequalities.
- To clarify objectives and priorities for network delivery in line with operational capacity, financial parameters, and contract requirements
- Lead on the development and implementation of integration with other services and stakeholders including Integrated Neighbourhood Teams (INTs) & PCN
- Lead the Integrated Neighbourhoods Teams Programme in the PCN, including the development of strategy and priorities.
- Promote the engagement of all PCN practices and act as an ambassador for the network.
- To proactively build relationships with system leaders to help influence proactive and new models of care.
- To use business intelligence tools to aid data-driven decisions.
- To lead the development and implementation of Digital Transformation bids
Operations
- To provide oversight and accountability to the delivery of PCN contracts and specifications
- Coordinate training and development of PCN staff
- Identify and record risks and issues and develop contingency plans with Network Leads
- Ensure all relevant national strategies, policies, targets, and frameworks are fully implemented
- To drive issues through to resolution, acting as a point of expertise
- To implement network-based projects
Finance and Governance
- To create and maintain accurate documentation in relation to finances, Standard Operating Procedures, Contracts, Contacts and decision making which is accessible centrally.
- To support the collation of financial information.
- To monitor and check the accuracy of the submissions for the DES and contractual requirements.
- Compiling and presenting financial reports.
- To ensure that the PCN's physical and financial resources are managed and maintained for maximum effectiveness.
Digital Infrastructure
- Improve digital accessibility for Rutland Health PCN patients.
- To ensure the PCN maximises its business intelligence tools.
- Develops a continuous improvement approach to digital accessibility for our patients and workforce.
- Creates data packs to support decision-making that includes but is not limited to demand, capacity, utilisation data, staff, and patient experience.
Relationship Management:
- To foster a culture of collaboration and promote learning to drive continuous improvement.
- To work as a flexible member of the PCN, providing support to other team members when necessary and working with the PCN management team.
- Work alongside the Clinical Director and PCN Board to deliver the DES and objectives set out by the board.
- Develop and deliver a communication plan using a wide variety of channels
- To take an active role in the development and embedding of the PCN's culture, values and reputation
- To represent the network at external meetings
HR and Workforce Planning
- Support recruitment, induction and training of new PCN staff
- Support recruitment which is aligned to a workforce plan
- To facilitate progression for the PCN staff to promote retention
- Undertake performance reviews and conduct annual appraisals
