Job summary

The Rutland Health Primary Care Network (PCN) First Contact Practitioner (FCP) will work as part of a multi-disciplinary team and have a key role in supporting the PCN to deliver a musculoskeletal specialist service to its patients. You will complement and support the existing teams of clinical staff in each practice, attending clinical meetings and MDTs where necessary. You will be supported to introduce new ideas and novel ways of working that will develop the FCP service.

The post holder will be empowered by the PCN to lead the FCP service and shape the way that it is utilised by our practices. They will also support the PCN if it looks to grow the FCP service by providing advice and assisting the recruitment process. The role is pivotal to improving the quality of the service, and the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.

Main duties of the job

Key Objectives

  • Provide pro-active leadership on the FCP service to the PCN, practice multidisciplinary teams, patients, and their carers
  • Develop and deliver patient facing services and clinics which improve patient outcomes, reflect the needs of the practice population and shift workload from other disciplines
  • Improve general practice/clinician understanding of, confidence in, and compliance with the scope of the FCP service
  • Provide musculoskeletal consultations as an integral part of the multidisciplinary team by triaging, assessing, and managing a diverse caseload
  • Develop yourself, the role, and service through participation in clinical supervision, training, and service redesign activities
  • Deliver training, mentoring and guidance to other clinicians and staff on the FCP service

Working with our 42,000 patients, we work collaboratively with Rutland County Council and community services to focus on local health needs and population health.

  • Develop and deliver education sessions and training to clinical and non-clinical staff on musculoskeletal issues
  • Deliver education and support to individual patients and patient support groups on MSK
  • Assess emergent clinical evidence and develop strategies for implementation in practice
  • Be actively working towards the FCP accreditation

Quality

  • Ensure compliance with CQC and other professional standards
  • Promote a culture of constant improvement and excellence
  • Conduct regular clinical audits and other improvement activities
  • Assess own performance regularly and make suggestions of how to improve personal and team performance

Health & Safety

  • Assist in promoting and maintaining your own and others’ health, safety and security as defined in each Practice’s Health and Safety policy. Ensure that any hazards in your working area are reported immediately to your line manager.

  • Ensure patient and Practice information is kept confidential and secure at all times, in accordance with the Information Security and Confidentiality protocols.

  • Training sessions to ensure administrative, clinical and computer skills are kept up to date with current technology and practice.
  • Regular appraisal meetings.
  • Regular team meetings.

  • The post holder will have a leadership role in supporting further integration of general practice with the wider healthcare teams to help improve patient outcomes, ensure better access to healthcare, and help manage general practice workload. The role has the potential to significantly improve quality of care and safety for patients.

There is one position available of 10 sessions per week. The salary is £45,000 per annum depending on experience. There is access to the NHS Pension Scheme, with 5 weeks annual leave plus bank holidays.

  • Commitment towards continuing professional development in accordance with requirements of ARRS and the FCP role
