Job summary

A fantastic opportunity has arisen for a fully qualified, experienced ANP to join our friendly, high achieving Practice Team. Experience of working in a Primary Care Setting is essential. The successful candidate will have achieved Masters level, or be currently studying towards Advanced Clinical Practice with significant proven experience of working as an ANP (minimum of one year) with a minimum of 5 years post registration experience. A sound knowledge of current nursing issues both nationally, regionally and locally as well as a good knowledge of NHS issues at the current time, is essential.

Main duties of the job

To provide nurse led triage and management of minor illness, minor ailments and injuries as well as chronic disease management for patients with long term conditions and complex needs. To be able to demonstrate clinical examination skills, with proven interpersonal skills and experience of conflict management. Ability to work well under pressure, to prioritise effectively, as well as ability to order own investigations and interpret results. Visits off site will be an essential part of the role as well as the flexibility to meet the demands of Primary Care working across 3 branch sites at Wickersley, Ravenfield and Braithwell. Use of own vehicle is essential to this role.

About us

We are a well regarded high achieving Training Practice with excellent QOF results. Currently upwards of 11,800 patients spread over 3 sites, based at Wickersley, Ravenfield and Braithwell. We are an SystmOne practice with a clinical team comprising of 5 GP Partners, 3 Salaried GPs, 3 Practice Nurses, 1 ANP, 1 Specialist Nurse for LTC and 4 Healthcare Assistants. We provide a range of services including Minor Operations and Contraception Services including coil fits and implants. Our Braithwell site provides dispensing services to its registered patients.

Job summary

The post holder will demonstrate courageous and critical thinking and is a caring, compassionate and committed experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will commit to demonstrating critical thinking in the safe clinical decision-making process. They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

Key responsibilities

  • Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
  • Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
