Job summary
One of the main aims of this role will be to help to support individuals to manage their health and wellbeing in their own homes for as long as possible through the provision of general health care checks and a phlebotomy service.
The Frailty and Phlebotomy Care Coordinators will deliver a phlebotomy service and general healthcare assessments for housebound patients across 3 practices on behalf of North Oxfordshire Rural Alliance (NORA) PCN. This will involve assessing required equipment and consumables from one of our practices, delivering the phlebotomy service and general healthcare checks in the patients own home and liaising with the patients GP if there are any health concerns.
Each job-holder will be required to work two days per week with a half day worked alongside the other job holder to complete shared administration and to ensure case-load continuity. The job-holders will be required to share Saturday phlebotomy and frailty assessments work, each working on an alternate Saturday basis.
Main duties of the job
The post holders sharing this role will be expected to work closely with practice teams to coordinate the delivery of phlebotomy and general healthcare services to housebound patients across the PCN
Undertake basic assessments such as taking weight, BP, foot checks and pulse rate to ensure that any housebound patient seen is receiving sufficient care and to contribute to the management of their long-term care.
Deal with a vulnerable group of patients with the potential requirement to highlight those who have limited social/health support and to signpost them to available community services
Learn the roles of the different community health and social care professionals to connect patients to available community services in order to obtain the appropriate care and support they require.
To be successful in this role you will need to have a strong track record in providing excellent general health care and phlebotomy services. Previous experience of care in the community is not essential but is an advantage. This is a developing role and so there will be the opportunity to shape this role to suit the needs of the community. Previous experience of job-sharing would also be an advantage.
