Job summary
As
a Health and wellbeing coach (HWBCs) your job will involve predominately using
health coaching skills to support people with lower levels of patient
activation to develop the knowledge, skills, and confidence to become active
participants in their care so that they can reach their self-identified health
and wellbeing goals. You will work alongside our team of Social Prescribers and
Cancer Care Co-ordinator to support patients with their wellbeing.
Main duties of the job
Provide
one-to-one health coaching support for people with one or more long term
conditions, based on what is important to them, with the aim of: Improving
people’s knowledge, confidence, and skills-levels
of ‘patient activation’
Empowering
people to improve their health outcomes and sense of wellbeing
Preventing
unnecessary reliance on clinical service
About us
We are an established surgery in West Swindon. We offer a high standard of patient centred healthcare to the local community.
We offer our patients a variety of appointments including book on the day, book in advance, online, telephone and video consultations.
The surgery is a member of the Brunel Healthcare Group and part of a primary care network alongside Ridge Green Surgery, North Swindon Practice and Park Lane Practice. Together we are committed to ensuring patients within the local community have access to the best care possible within the resources that we have.
As part of the primary care network we have recently improved services by jointly employing a primary care Pharmacist to improve the quality and safety of prescribing, and also a Health and Wellbeing advisor whose role is to coordinate social initiatives and advise individual patients how they can improve their health through lifestyle changes.
In addition we also offer additional services including, Physiotherapist, Medication reviews, Diabetes Management, Blood Tests, Antenatal Care, Family Planning and Chronic Disease Management.
