Job summary

A Practice Nurseplays a key role in delivering high-quality primary healthcare services, working closely with patients, doctors, and other healthcare professionals. In this role, you will be responsible for providing a wide range of nursing services including health assessments, vaccinations, management of chronic conditions such as diabetes and asthma, and wound care. You will also support preventative healthcare through health promotion, patient education, and screening programs. Strong communication skills, clinical expertise, and a patient-centered approach are essential to ensuring the best outcomes for our community. Main duties of the job

The main duties of the roleinclude providing high-quality nursing care to patients, such as administering vaccinations, including baby immunisations, and performing health assessments. You will be responsible for cervical screening tests, managing long-term conditions like asthma and diabetes, and adminstration of travel vaccinations. Additionally, the role involves promoting health education, offering lifestyle advice, and supporting patients in managing their overall well-being. Collaboration with GPs and other healthcare professionals to ensure a seamless patient experience is also essential.

The role involves providing high-quality nursing care to a diverse population of approximately 7,500 patients. You will play a key part in delivering a range of primary care services, including immunisations, chronic disease management, and health promotion, while contributing to the ongoing success of our highly regarded practice.

Key Responsibilities:
  • Immunisations: Administer routine and seasonal vaccinations, including baby immunisations and travel vaccines.
  • Cervical Screening: Carry out regular cervical smears in line with national screening programs.
  • Chronic Disease Management: Provide ongoing care and management for patients with long-term conditions such as asthma, diabetes, hypertension, and COPD.

  • Womens Health: Provide care related to family planning, contraception, and menopausal support.
  • Health Assessments: Conduct new patient checks, NHS health checks, and routine health assessments.
  • Collaboration: Work closely with GPs, healthcare assistants, and other members of the clinical team to ensure integrated, patient-centered care.
  • Infection Control: Ensure the highest standards of infection control, maintaining a safe clinical environment.
